Med student trying to refine my reviewing habits here. Tons of people keep on recommending Anki for studying and I tried getting into it multiple times but it really doesn't help me form reasonable chunks of info I can retrieve when needed. I know I may be doing it wrong, but I really, really prefer the classical personal note taking method. I feel it helps me learn better. My OCD isn't letting up about whether or not I should still use Anki though.

Any person in /med/ who studies the same way as me? Is Anki really that necessary? Of course, I'm only like this because I want to be the best possible doctor I can be.

I would greatly appreciate some advice. On Sunday, I travelled from country A, a rich country that has introduced measures to prevent the spread of coronavirus, most people wear facemasks in public, there are temperature scanners before entering large stores, to country B, a third world shithole that has barely any preventative measures against coronavirus. While travelling I observed all of the new safety regulations airlines introduced against coronavirus, negative PCR test before travelling, wearing a facemask while travelling, getting tested many times for high temperatures at the country A airports, etc. But in the airport of country B, most of the airport workers were not wearing facemasks, some of them were standing around less than a metre together, and the next day I went to a hospital in country B, while I myself wore a facemask( not the N95 mask, the pic related one), most of the people in the hospital, doctors, patients, receptionists, were not wearing facemasks, and they were also standing around close together, and none of them were tested for high temperatures before entering the hospital. There are thousands of confirmed cases of coronavirus in country B, and that is with the few tests that the health ministry can afford to perform.

My question is, is there a chance that I was infected with coronavirus in the airport or hospital of country B, and should I wait before I want to visit grandparents in country B, even while wearing the pic related face mask? How long should I wait before visiting grandparents? I don't know of any places where I can get a coronavirus PCR test in country B. An important point is that I have heard of hospital workers in country A testing positive and accidentally infecting others with coronavirus, so hospitals in country B must have a worse situation, right?

If anyone could give me an advice regarding my heart rate I would greatly appreciate it.

Every time I exercise my heart rate goes stupidly high and I don't know what to do. Today I went for a run and few times I measured it it went slightly above 200 (I'm 21) after 1km. Pace was around 6 min/km. After 250m of walking the heart rate went to around 160-170. After another 1k heart rate again raised to above 200. Then after I finished running it stayed 150-160 for probably more than 20 mins. Sometimes my HR is high when I stand up, even when I'm not doing anything, could be around 120-130. It only takes for me to bend and heart rate drops to normal 70-80.
Probably worth mentioning that even though my HR goes above 200, I can feel no symptoms at all. No pain in chest, no SOB etc.

I've seen cardiologists, electrophysiologist. They said it's "arrhythmia caused by sensitive autonomous regulation" (sensitive sympathetic nervous system to be more precise). Respiratory arrhythmia is very noticeable. I failed veloergometry (exercise stress test) 2 times (because I get scared really quickly, heart rate was simply too high (at around 150) to begin the test). One time I passed veloergometry test, though physician said HR ramped up a bit too quickly, but apart from that, nothing unusual was seen.

Basically everything I got tested for was good except for vitamin D (53nmol/l). Could vitamin D be the cause of this?

To cut a long story short, physicians said I'm fine and there's generally nothing to worry about. Though I really want to believe them and even though they are very good and competent physicians, is it even safe to exercise at HR above 200? Anyone has any advice on how to approach this problem and how should i try to solve it?

You have to take tests or read books regarding sensorial-learning types. I'm a corporal-kinesthetic learner so I react more positively while flipping/typing/writing/doodling etc. It also means I'm kinda retarded so I don't have a large time of undivided attention; I need to take constant breaks and shit.
Anki is good to memorize key concepts. You won't correlate your learning or have any deep knowledge from it.
Right now I'm studying for the spec exam and Anki has helped me to memorize key concepts like GSs, gradings and so on.

Just talking from personal experience - I'm healthy and could comfortably get my heart rate up over 200 in my early 20s. Never had any issues with it. I will likely always live insecure of my intelligence because I failed out of pre med. I lived a stunted life after that because it was presumed my socioeconomic status would remain low so I didn’t get many opportunities both in my personal life or professionally. I’m kind of bitter of people in healthcare but also jealous. Worst way: use a prebuilt deck like some robot and memorize a bunch of disconnected meme facts for nothing

Best way: get a collection of textbooks on the core topics of medicine for your preclinical years. I used Robbins Pathologic Basis of Disease (the big one), Golan's Pharmacology, Harrison's Internal Medicine and a book on clinical examination (use whatever is standard for OSCEs in your country). Then when you are learning a specific topic at university, look back at your textbook and read the relevant textbook pages and chapters. From both your lecture slides and your textbook, make Anki cards. Use cloze style cards and image occlusion as much as you can but "basic" flashcards are good as well. Don't be afraid to memorize lists and tables. Make your own lists, tables, classifications and incorporate them into your flashcards. For example, if you want to memorize all of the cellular physiology relevant to catecholamine pharmacology, make a table where each row represents a receptor (e.g. beta1 receptor) and columns that represent the distribution of the receptor around the body, agonists, antagonists, intracellular G-protein, second messenger etc. Then take that table and use IMAGE OCCLUSION.

The essential part of this strategy is using a textbook and treating it as the absolute authority. Use your lectures as a way to introduce yourself to topics and glean extra nuggets of information. By making cards yourself you reinforce the concepts you learn and will put you in a much better position than those who use pre-built decks as you actually learn the stuff properly, and at the same time by using Anki you actually retain what you learn so that you can speak and present your knowledge with confidence. The traditional kind of studying might have worked well 20 years ago when (1) there was less to learn and (2) our attention spans weren't destroyed by the internet.

I don't think an isolated low vitamin D could cause what you've mentioned. Also be aware of the fact that a large portion of the population is "deficient", just eat more animal products or get more sunlight, not a big deal.
There are some maybe obvious medical therapies (e.g. beta blockers) that would help you, but the problem is that the doctor doesn't want to give you medicine if you aren't actually sick. Non-medical therapies are what you should focus on, someone mentioned meditation which may help you increase the vagal tone on your heart. Probably the best recognised way to increase vagal tone naturally is to become fitter, which unfortunately for you seems difficult, if you can find a way to increase your cardiovascular fitness with your condition then that may help you. Other things you can try include: cold showers, fish oil and deep + slow breathing (i.e. meditation).

It occurred to me that my peptic ulcer might be relevant. A stomach ulcer can cause deficiencies in b1 b2 b6 b12 c and folate -- water soluble vitamins. Come to think of it my muscle weakness began presenting within about 8 months of first presentation of the ulcer -- little things like it felt weird to jog and I could no longer perform the routine muscle up to get to the store as at that time used to be easy -- and didn't come to completion until the beginning of this year. So it's pretty much for sure a vitamin issue at this point.

What are the chances that after I resolve this vitamin issue, that I will return to normal? Will any lesions/scar tissue/masses likely be left behind that would prevent function from being fully restored with stem cells?

Not possible and not really effective. There are multiple types of memory, one of which is stored in the "body", close to motor output. If you wipe your memory you will still be miserable, if not at first, then eventually. You just won't understand why. Essentially, all you have to do is learn how memory is stored for you. Then draw the memory in mind, break it down to its underlying associations, and recursively strip them out until there is nothing left to trigger the conscious recollection. Repeat. It's not permanent however, you should pursue ECT for that.

A question for med anons

So I'm kinda retarded/air headed, have the attention span of a cat at a laser party and have little mental stutters that cause me to butcher jokes, spill spaghetti instead of delivering cool opportune one liners and repeat big chunks of sentences unconsciously.

I was perscribed adderall when I was younger to help me focus in school, but I only really got an hour/hour and a half of good focus before i crashed and the side effects kicked in and beat my ass. It also made the stutters alot worse after it reached its peak.

I no longer take it but I noticed that I get similar levels of concentration when I drink alcohol or take in sativa dominant strains of cannabis.

Terrible study method. I did the same thing for a month and did well on tests but it was miserable. Ultimately, I found making notes, tables, diagrams, etc to build connections and understand key concepts then supplementing with a premade deck the most efficient. Medicine isn’t just about memorizing random facts on a topic. Understanding how ideas come together and building a strong foundation is what really sets you up for success long term. Everyone forgets the small details and they’re easy to fill in when you review later as long as you have the basics down. Also, it is possible to do well on a test without really understanding the topic.

Yeah the premade deck won’t be comprehensive but it will cover the essentials. Anki really shines with long term retention anyways. You’ll separate yourself from your peers when you’re remembering minor facts 2-3 months past a unit. Assuming you stay caught up with your reviews. There’s also an issue about how you make your cards. Cloze deletion “fill in the blank” is much faster than active recall.

As a med student, I hate the white coat. It’s old school boomer and looks overly formal and out of place since so few doctors bother with them now. More importantly, in the U.S. at least, a lot of other medical specialties started wearing them so it’s lost all meaning. This includes NPs, PAs, pharmacists, even some nurses.
