I'm essentially dying, doctors are useless, so I'm now a dying monkey with a potato (microscope) and I have no idea what I'm doing.
I did a few smears a few weeks ago before my shit completely crashed, and I saw lots of echinocytes, but this specifically looks different to me and only started showing up today. There are a few examples in the video.

If you have no other options try to get some exercise.
I'll give you the barebone that helps to stop decayment caused by shitty lifestyles.
It's completely anectodal but maybe it can help you at least going further and figure out how much is lifestyle and how much is an actual issue.

1) Walk 4km a day
this alone made me healthier, I noticed this because during vacations of complete laziness my health often decays, while when I walked to work and back I was always in good shape.

2) Avoid sugars
salty is superior anyway, you can eat sugary stuff once a while but you should absolutely avoid sugary drinks. Drink only water.

3) If you use use belts or other things that tighten your waist, try loosening them up
I have great suspicions that headaches are caused by this, and headaches may be due to light clots that form thanks to the tightening. I have several reason to think this but it's offtopic.

4) Meditate
Stress and negative emotions take a hit on the health, not because of magic bullshit, but because the brain has to produce chemicals to make you feel those things, and those chemicals do bad if they go too much too long. Try to meditate at least once a day up, 15 min each session. Better if twice.

5) Eat a balanced diet
A little of everything. If you're fat this is the chance to lose weight. The trick to lose weight is to keep your abdominal always contracted.
Just keep contracting it until you do it subconsciously forever. You'll feel fed up much faster.
Also learn the difference of being sated and having a full belly. You only need the first, a full belly means you fucked up.

That's it. Do these five things and see what happens, best case you get back your health, worst case you've cleared up lifestyle issues and doctors may be able to see something they couldn't before.
I'm only writing this because I've experienced these things first hand from my sedentary shity lifestyle, maybe it can help you or someone else.

I was in absurdly good health before this all started 12 years ago. I was running a marathon every week, on top of rock climbing, mountain biking, etc. I was eating probably the pinnacle of good diets at the time.
Shit just crashed almost overnight and I never got better.
I have zero appetite, most foods seem to cause adverse reactions at this point, and I can barely get around the house most days.
I'm 6'5", normally 180lbs when I'm feeling somewhat okay, in good physical condition. When I occasionally have a good day, I can easily run 20k without any issue, but most days I'm essentially a 120yo man with the worst aids ever.
It spontaneously resolves completely, but only for a few days at most, maybe once every year or so.
I can't exercise (I just pass out) except when I'm in remission. I don't really eat anything with "sugar" other than ketchup and a little in kombucha and blueberries. I definitely can't eat a balanced diet anymore, except on the rare good days.

I've lost 20lbs in the past two weeks, and that was while I was still making my 2200 calorie per day requirement. I've been barely able to get 1000 for the past few days.

The original issue definitely wasn't lifestyle, and the symptoms haven't changed since my lifestyle changed. When I'm asymptomatic, I'm groin grabbingly active for as long as I can be.

I keep coming back to fungal infection, and/or kidney/liver disease as well, and I do have a bunch of diagnostic markers to suggest mild renal and liver disfunction, but they both spontaneously disappear every week or so.

I don't really exercise at all anymore. I've gone months and even years without. No change. When I feel abnormally good, I can work out. When I don't, I can't. It's not even a matter of trying to force it. I just black out immediately if my body doesn't have the ability to do any training.

That’s if you consider African protozoan parasite to be considered the true cause of aids. Lots of aids theorists.
