mandibular blocks are common for anesthetising lower teeth, why does nobody do maxillary blocks?

also general question for medics/anaesthetists:
is local anaesthetic still required for patients under general anaesthetic?
my friend went in for surgery and even though he was put to sleep, he still had a local anaesthetic injection. why would you need local?
