What are some huge mistakes you've made while attempting to solve a question?

Reading the question wrong and doing more than asked resulting in less time for other questions. Now I always use a highlighter to highlight key parts of the question on exams so I don't fuck myself over. Literally created a shear and moment graph with their respective equations when all that was asked was to find the shear and moment at one point.

Overthinking it.
Underthinking it.
Forgetting the writer of the question.

Spending too long on stuff that doesn't benefit me.

Forgetting to see the bright side.

Forgetting to go outside and take breaks.

Forgetting to file my taxes.

Forgetting to attempt to re design the government.

Just kidding.

Not exercising enough.

Forgetting to call my mom and tell her I love her.

Consider the relation defined by R = {(x,y) ∈ a+ x a+ | x+y is a prime}

Prove or disprove: R is irreflexive.
Suppose R is reflexive. Then x+x = 2x must be prime, but 2|2x, contradiction, ∴ R is irreflexive.

That a+ is supposed to be R+ (positive reals). I don't know why that happened.

Bonus.

Determine the recurrence relation given the explicit formula.

My friend and I both made this mistake on the same question at the same point on the exam when we sat multiple desks away.
