That's normal for the RS-68, it dumps hydrogen overboard before ignition to chill down the engine components, and that hydrogen catches fire once the engine ignites. The heat then scorches the insulating foam. This is one reason why the Ares 5 rocket was cancelled, it was going to require the RS-68 to be man-rated.

Let me put it this way.

The launch vehicle did not fail. Everything that was in SpaceX's control went perfectly. Northrop-Grumman requested that not only they build their own payload adapter and decoupler, they also insisted that they themselves integrate the payload and close the faring, not SpaceX, because muh secret satellite. The Falcon 9 launched, staged, burned to orbit without a hitch. The payload then failed to decoupled because N-G didn't trust SpaceX's hardware and wanted to do it themselves. The satellite was therefore dead in the water and the order was given to deorbit the second stage with the payload still attached.

SpaceX literally did nothing wrong, except maybe by letting N-G do what they wanted.

What I care about is knowing whether the launch actually was a failure or not. Sure, I'd prefer if it actually was a success, but that's not why I doubt the assertions of failure, it's because of the secrecy of the mission and the statements that don't quite add up.

Usually the job of SpaceX also includes payload deployment, but in this case someone else had built the payload adapter and fitted the payload themselves, so it was pretty much their show at that point.
