Assuming you have at least a 3.6 math gpa or so, these are in my estimation the things that matte from order most to less important:

Letters or recommendation. This is by far the best way for the professors at whatever school you're applying to evaluate you, from the mouth of another professor. Ask your professors if they can write a strong letter, not just a letter or rec, and try to get to know them. Hearing from another professor at a good school that so and so was impressive or stood out for such and such reason is more valuable that a bunch of stats on paper that vary from school to school.

Research. I think a lot of people overrate this, most undergrad research, even by strong students, is not really anything other than an exercise in how to study independently. However, pretty much everyone applying to grad school will have research experience and so should you, do and REU or work with a professor, it's a good way to go give your professor or REU professor something to write about in a LOR anyways. I did a couple REU's, they were fun and yielded a good letter, as well as some good experience learning how to work on a problem independently.

Preparation/drive toward a certain field: obviously you should take as many upper div and grad courses that you can. Even better, if you have a good idea of what area you want to study, and that you focus on it. Not overly specific or anything, but if you want to do analysis, you can take Measure theory reals, then Complex and then Functional or Harmonic (or just study it on your own if you can't take all those classes), and you will have a clear head start when you begin and write your statement, you will seem like you know what you're doing, and you will be able to apply to schools that are strong in your particular field, rather than hoping you end up liking the areas that some random school you get into has.

I forgot to mention the GRE. It's just another hurdle to jump through. The general GRE is a joke and you'll do fine. The Math GRE is annoying, you basically have to train yourself to do the problems quickly. About 40%-50% is regular calc, then there's some linear algebra, abstract algebra, non-measure theory analysis and complex analysis. The last 4 subjects will maybe make up like...12 or so questions of the 60 question test, so make sure you can do the calc and linear algebra quickly (I think there's also some basic plane geometry and algebra as well). There are various GRE's from previous years floating around the internet as well as books, if you practice them they shouldn't be hard.

Top 20 should be doable. Top 10 it starts to get pretty intense (but still possible). At that point a lot of people are at the level were they're maybe published and done some non-trivial research and they all have more or less the samet applications, but I don't really know so I'm gonna stop talking out my ass.

Rankings aren't irrelevant, but what really matters are the professors at the school. Some schools may be ranked meh, but very strong for a certain area (the people who do rankings are generally idiots), and working with one of those well-known professors at a ~30 school may be better than doing an average phd at ~15. i.e. if you want to do analytic number theory, you might rather go to UCLA (Terry Tao) than Harvard or whatever (I don't think Harvard even does analytic number theory).
