Because hearts pump all the blood, kidneys only pump the part of the blood that's not pee. If you only had kidneys instead of other organs the pee would just accumulate until you were a kidney pee monster.

Looks like a giant aneurysm on the left main coronary artery. That by itself is not an indication for transplant, so there must be something else going on.
