Is it possible to punch someone so hard in the gut that you actually break through their skin, and are then able to pull out their internal organs?
how hard would a punch have to be to do this?

It's easier to rip/tear skin than it is to punch through it.
Get some spiked knuckles and you may find it easy to break skin with a punch.
If applied to a small area, it doesn't take a whole lot of force to split skin and the begin tearing it from there.
This is the basic idea behind knives.

No, it is not possible. It takes a fair amount of effort even to stab a moderately sharp knife into someone. Your hand has thousands (millions?) of times the surface area, requiring commensurately more force, in addition to the fact that you have to go deeper in.

Cuts often occur during boxing matches.

A hard fist contacts a hard face (thin skin stretched over firm bone). Sometimes, the punch lands with a twisting motion (a lot of coaches teach this technique to create cuts). The skin tears, and the opponent starts bleeding.

On the other hand, a hard fist hitting a soft abdomen (even one with impressive 6 pack abs is softer than bone) will be absorbed into the flesh and body tissue.

I have seen many people die from internal bleeding secondary to abdominal trauma. The evisceration of the abdominal contents is extremely rare. I've seen one or two car wrecks where this happened, but most of the time, when someone gets hit hard in the abdomen (hard enough to tear the descending vena cava or rupture the spleen/liver/etc) the skin remains pretty much intact.

Based upon my boxing and medical experience, I would have to venture a guess that a very,very powerful punch coupled with some sort of sharp-pointy-razor-glove would be necessary for OP to attain his goals of one day holding aloft the spleen of his defeated foe (because I know full well that's why you asked this question)
