Don't know about Japan, but here in the US it's overbloated budgets, literally zero documentation from the 60s, literally zero documentation to this day (you'd be stunned at how few reports are written in the aerospace industry), companies are a bit more let's just say greedy for lack of a better term (basically they're very protective of their designs compared to historically during the cold war), there are a billion mismanaged processes on the government side because government employees are midwits (anyone actually skilled moves to contracting or at the very least goes to an FFRDC), it just keeps going on like this.

Oh also, the budgets are so fucking large for anything space that there is even more waste fraud and abuse going on than typical for these literal tiny box systems.

Misplacing things isn't destroying them. Having no means to improve it's archivql or storage isn't desteoying. The Viking Mars mission data magnetic tapes have been slowly degrading and that data has been lost over the decades as well because there was no funding for a project to convert that data onto new media forms. Just an example.
Doing things right costs money, money that sometimes just isn't allocated towards NASA in sufficient amounts to properly store and maintain organized archives for the future generations.

Competency crisis in the West. Browns are flooding into industry and institutions, and the effects are being felt all across mainstream technology and engineering. Everything in high-tech and advanced engineering is getting worse; things still advance, but much more slowly, execution fails more often, efficiency goes way down / costs way up. To give you an isolated example of a competency crisis — the Venezuela oil industry. The majority population took over and the new government gave the oil industry into the care of their majority supporters (brown people) — and they proceeded to run it into the ground, destroyed the industry’s productivity — because hydrocarbons is actually difficult and takes high IQs to run well. The reason why the U.S. transformed into an oil super power again (for a while) was because high-IQ engineers and scientists figured out how to do highly productive / efficient fracking while it remains alien technology for most of the world.

Name any high-tech sector and you’ll find that it’s both led by the West AND currently experiencing a downfall. Competency crisis will eventually crash the West and lead to overall progress of technology grind to a virtual halt. Without the Western IQ the idea of landing on Mars will be a dream within a fantasy. Our future is a global peasant dependency system ruled over by a handful of oligarchs and white IQs will be relegated to middle management to what extent that will remain.

That turns out not to be the case.
When the USSR fell and access to Russian archives became possible, the whole "We were never trying to go to the moon" bullshit was exposed.
The Russian program failed for the ironic reason that while the regimented state-runs-everything Soviet system was funding competing lunar programs, splitting available resources between to rival agencies, the US tan a single focused program, allowing them to put all of their larger pot of money into one unified program.
The death of SPK didn't help them, either.
