What he means is that while some excess deaths can be explained by a lack of general medical care during the COVID era, that excuse becomes less true as time goes on because dead people only contribute to excess deaths numbers once. The fact that excess deaths remain high excludes the possibility that delayed care can explain the increased death numbers.

The expected trend is a large spike in death during the pandemic followed by a dip in excess deaths post-pandemic (because many people that would have died this year died lastly year instead) that regresses to the mean. The lull would be slightly reduced due to a lack of care during the pandemic (acceleration of disease due to lack of screening or treatment), but regression is still expected and the data doesn't seem to reflect that.
