I recently had a surgery and there was this really cute 10/10 qtpie Asian nurse that was there with me entire time. We were talking a lot post-surgery and I lowkey felt like she was flirting a bit. Complimenting left and right, laughing a lot etc. And I was partially still under anesthetic influence so I was going at it as well (Which I wouldn't while not under influence because Autism). Hand fed me food when I could have eaten it myself (It was a minor surgery, I was fine to eat). She was quite touchy, and being very nice. Then when I got up she again complimented my tall stature and large physique, walked me to where I was going, and on the discharge documents attached a post-it saying "Hope you feel better soon <3" I had a boner the entire time even though I just had surgery ngl.

Now, yes, I am very affection starved so you don't need to say it. But was she into me or are all nurses just this nice. I had quite an ego boost from that after, but thought it might just be general demeanor post-surgery for nurses, since I haven't had a surgery before. But nurses outside of that haven't really been as nice. Nice, but not to this degree.

So was it real? Or am I deluded?

I posted this in another thread but I wanted to post it here too.

What are everyone's thoughts on AI's impact on the need for doctors and the role of midlevels? I think the jobs for doctors will be massively reduced due to AI augmenting the diagnostic abilities of physician assistants and nurse practitioners. We're already seeing hospitals hiring more and more PAs/NPs per doctor, and NPs have achieved the right to practice independently in 26 states.
PAs and NPs are continuing to lobby for more independent practice rights in more states, and if AI can make their diagnostic ability comparable to that of the average doctor (it basically already can), then do you really think hospitals will keep hiring doctors? The only safe specialty will be surgery, but that will become overwhelmingly competitive (more than it already is).

The only saving grace for current doctors will be if they're willing to take much lower salaries to compete with PAs/NPs, but that isn't an option for new MD grads with $300k of debt. This is actually a good thing for everyone except MDs, as it will bring down the cost of healthcare significantly.

I feel like anyone who wants to be a doctor should pursue the PA/NP track, otherwise they're going to get screwed.

Also the premise here isn't that the AI is doing all of the work. The premise is that an NP, instead of consulting a doctor for a more complex/rare diagnosis, will consult the AI. a lot of reasons. im not into surgery too much, was worried about trying to get into the very competitive ophthalmology spots, scope of practice keeps expanding for optometry for nonsurgical interventions (lasers being the new big push in many states though i don't know if i'd ever see enough patients where i plan on practicing if it's worth to buy one), setting up a private solo ophthalmology practice seems incredibly more difficult/headache than solo optometry, optometry teaches more about vision therapy which i found interesting (i've heard many ophthals still view it as shit), the amount of schooling, etc.

Most misdiagnosis happens in common medical disorders that present in a very unusual way, not the other way around. The presentation of stroke, heart attacks can be extremely variable. A patient with shoulder pain,stomach pain or toothache as their only symptom can be a heart attack. It is usually experience, careful examination and observation that allows you to make the distinction.

Nothing stops NPs from gaining that experience. MDs fresh out of medical school certainly don't have it. If you took an MD track and cut out a lot of the memorization that's going to be made obsolete by AI, put more emphasis on clinical skills, and paid residents a proper wage, you'd have yourself a PA/NP program.

I can't speak for w. europe, in ex-ussr shitholes there's a clear delineation of education and responsibilities of the two
nurses work with their hands, doctors work with their brain
doctors do the diagnostics and prescription, nurses handle the actual medication administration and patient care
it's a bit more muddled in surgery and anesthesia where nurses are trained in some basic skills to assist the doctor(i.e. the operating nurse might help suture or the anesthetist may have some basic airway skills), but all the advanced and risk-involved tasks are still doctor only

1. guidelines apply to about 80% of the patients. often a person comes in that doesn't fit the gudielines.
2. guidelines is what's minimally expected.
3. most important point. Guidelines DO NOT tell you the effect size of the treatment that is recommended. If you read the papers you'll see that a large part of the treatments we use have tiny tiny effect sizes. Having an idea of those tiny ass effect sizes is great when you have to make a clinical decision and actually not be distracted by nearly meaningless interventions and just focus on the important interventions that may actually.

I had my covid19 vaccines (3 or 4 shots total, I can't remember) and encouraged everyone to get them because the vaccine (in particular) was a good thing and helped us get out of lockdowns. I have compatible symptoms except visible bleeding or nausea but a colonoscopy and a gastroscopy were negative so if it's there it's just in the small bowel. Also, nothing has been seen with ultrasound yet.

The doctors have told me to have an enteric MRI but I am afraid of the gadolinium deposition consequences in the long run form the IV contrast, many studies are being published warning that even the "safe" modern gadolinium chelates do stay in your tissues including the brain and the long term effects of it are unknown.

On the other hand if i go for a CT scan of the entire abdomen the radiation dose would be very high.

Could any anon provide any insight about my predicament?

I traveled to go to Hopkins hospital for the surgery, so one of the biggest hospitals in the world. It's not richfag per se (I think, there was a lot of black people there lol), but I think much of the staff thought I was rich because of some things, incl. the nurse (I think she even said something about how she was impressed and that I must be smart). Even though I'm not. Not rich at all, nearly broke. So maybe it was just because of that.

Depends on your cut-off, just "elevated" calprotectin like above 50 has low specificity as you say. However above 200 in the presence of clinical symptoms should be sufficient to warrant more testing. Especially given how often crohn's disease is only restricted to the Ileum alone. I'd suggest doing the coloscopy again and request a ileum sample this time.

Even a very high cut (>250) off has a poor specificity (0.49 for endoscopically confirmed IBD when audited locally in our service).

Of course a high calP needs investigation (as has been done in this case), but it is very far from synonymous with IBD. It's a classic exclusion test, like a d-dimer although marginally less shit.

Redoing a negative colonoscopy only makes sense if the ileum was deliberately not intubated, which may or may not be what happened and even then the diagnostic yield is going to be low for an invasive and potentially dangerous test.
