Hey fegs. I've got tummy issues/pains every time I "flare up" on some food, and a skin issue going with it.

Went to my doc yesterday and he instantly said; allergy. He prescribed me cetrizine for a week, and offered me a lactoce intolerance/allergy test(not sure which one he said specifically, I know there's a difference).

Anyways; I've had had multiple blood tests done, sent in my shit, did a swab test to see if I had harmful bacteria/fungi, had a patch test, and every single time, the results were negative.

Mind you, my diet isn't composed of shit, and I take zinc/d3. Watch my food like a hawk. How reliable are allergy tests? How reliable would a colonoscopy be, when I send in some of my gut tissue to a lab(this worked for a friend).

Gluten is a bit weird, there are antibody tests, genetic tests and the traditional gold standard: colon biopsy. Gene tests work in only exluding people if they are negative, positive results provide no additional diagnostic information. The EKG was the screening tool, you screened in for an MI and you got a troponin to confirm/deny that it was an MI. That is a different scenario. That is a patient who has a complaint related to MI or a complaint that is considered an MI equivalent (in elderly, women, and diabetics). Also, the exact timing is up to the facility and ranges from 6-12 hours. The likelihood of having an MI without an elevated troponin at 6 hours is very low so many places will only observe for up to 6 hours because they want to free up beds.

Of course, technically, you could follow the science strictly and say that if the patient had symptoms for over 6 hours and the EKG and first troponin are negative, you are safe to discharge because the recommendation is to get a troponin 6 hours after symptoms started. But that's rare because nobody trusts patients to pay attention to that stuff.

We know the automatic interpretations are inaccurate, but that's why I stated in multiple leads following a specific vascular territory. The reason to do that one troponin is to make sure there isn't another underlying issue causing some type of heart strain.

AFAIK, there isn't science behind screening with troponins in someone who is asymptomatic, so the general recommendation is to check a troponin. Not sure about where you practice but here in the UK anaesthetists can have a lot of freedom in how they manage patients. You can choose whatever induction agents, analgesia, pressure setting, airways and induction adjuncts you want as long as you take responsibility for your decision.

I know one consultant that did all his operations opioid-free and loved IV Lidocaine and just gives labetalol when the pts HR and BP spike due to surgical stimulus. Another one only ever does TIVA and titrates to BIS. Another loves regional stuff so does lots of local blocks with a little systemic opioid.

They all have different bits they like to do for specific pts as well e.g obese/paeds/geriatrics

While on university my IQ was measured by a psychologist at a 115-120 range. I'm not retarded at all, it just happens that I was neglected at childhood.
Both my parents worked so my only caretaker was a rural-bred cousin (f) that belittled and isolated me daily and didn't notice/cared about my intelligence.

This year I not only grinded my brains against the textbook but also was revealing cause I got the help I needed in order to have discipline and a learning methodology suited for my dominant intelligence.
Last year I got a 53 on an exam developed to measure the best general practitioner, this year a 61 on a test that changed its content to measure the suitability of the spec candidate (a lot of trauma and life support algorithms). We don't have to study the full books and they'll translate only the chapters of interest and make books based on those. The thing is that if you want to do residency in romania, you must know romanian. There are free (AFAIK) courses of romanian that the uni offers for foreign student. I have arabs and punjeets in my year (romanian program, not engl or french) and they're doing pretty well.

Iasi and Cluj are powerhouses but imo, Bucharest is also up there. I'm in Timisoara but do not recommend it one bit, I personally dislike it and wish I was in Cluj or Bucharest.

The residency options are pic related, plenty of field, no need to go IM-> subspec anymore, you can pick for example Cardiology and the fellowship is included in the residency.
