If it's your passion and if you think medicine will make you miserable and you're in a financially secure position to do so... Then do it

I've been thinking about switching careers from medicine lately too (senior med student here). Something about how this field doesn't seem like what I thought it would be, and the fact that maybe my interests lie in other areas. And it's made me think more deeply about the fact that we have only one life to live on this planet, so it's better to live it the way that makes you happy than to strive to some false goal. Obviously doesn't apply to everyone but yeah. Rambling on now so I'll stop.

I just don't understand how anyone can dislike young adult-adult patients who
a)can clearly explain their issues and symptoms and medical history, unlike babbies and senile/alzheimer boomers
>b-but talk to the mom/relative
i'd rather talk to the patient if i have a choice
b)aren't unpleasant to touch/can stay still and obey commands for manipulations, unlike babbies and senile old people
I'm just finishing my internship(non-American, i guess it's residency-equivalent) in anesthesiology, and 20-40 year olds are my favourite, because I can easily and clearly extract the information I need, and be sure that they understand my instructions in return.

You're right, not going to argue with that, but I simply like them much less than the elderly and kids. For me adults are from 30 - 65 (retirement).

The reason I probably dislike them, if I have to choose one, is because most of the boomers from 40 to 60 just discovered the internet because how easily accessible is nowadays and believe all the shit they read.

I know that the elderly can rant for hours and kids/adolescents lie a lot, but they're much more easier to deal with, imo. Usually old people don't have someone to talk to so they just need reassurance and a few nice words. Give kids some stickers and they'll stay put and treat adolescents like adults because that's how they want to be perceived.

It's really easy, I just prefer those over adults even if it's objectively easier to deal with them, but it's also kind of boring. Kids and old people make me happy.

I have been in lockdown since March.
I caught the cold twice, once at the end of May and once again now. This is normal for me. But this should not be happening during COVID lockdown, no?

I'm bald, 183cm but somehow I'm doing great with kids. In the beginning I thought it was going to be really hard but looks like it wasn't hard at all.

Kids from 4-10 are fine to deal with in a childish manner, like "Wow, that's a nice toy, do you like superman, etc" but from 12-18 when puberty starts to kick in, treat them like adults, it's really easy but they lie, so everything they say, you take with a grain of salt because they might be embarrassed.

For example, when I was on peds rotation, I had a 14 year old girl that complained of lumbar pain, Giordano was positive, but it ended up all being made up by her because she needed attention. Her parents were working in another country and she was staying with her grandparents, that's why it's very important to ask about relatives, relationship with the family, etc.

In the end, just don't try too hard and always smile to kids. My prof was a little old man in his 50s, the good old classic grandad figure, super kind and understanding but very strict on children's behaviour. (He's a pediatrician that specializes in Diabetes and metabolic disorders and every single morning, he'd go into the diabetic ward and make the kids open up the drawers of their nightstands and check for sweets and such). As I said, don't sweat it and be postive.

Part one of my textbooks/atlases/other. What's in green is /med/

>>11922133
Tbqh, I barely had any children that went ballistic while consulting them. Most of the kids were understanding and didn't fear me and if they feared me, I usually put my stethoscope on their leg and tell them "See, it doesn't hurt" and they would calm down.

When auscultating and they get uncomfortable if you take a long ass time to listen to their heart, then just move the stethoscope around a bit. I like to put it on their forehead and say "Wow, you're a smart one". It sounds super cringe now that I write it down, but it's what it worked.

So the short answer is to keep them busy. Youtube videos, toys, stickers and many more. Btw, always bring stickers to peds ward.

Bump? I mean I get medfags aren’t researchers but I mean even an opinion based on someone who knows how the immune system works would be helpful. Wait, do doctors even need to know the detailed mechanism of action of immunology or are they just taught how to treat? For example, I used to work as an electrician and you can wire up a house with zero knowledge of electrons and that was was in fact the case with everyone I worked with. Is the same relationship true of doctors and the medicines they use?

Are you asking if someone can have memory b-cells without significant serum antibody against Sars-CoV2?
I think so; not a doctor but I took immunology

Antibodies are products of effector immune cells, they aren't even cells.

It's been a while, but I think it's
antigen > t cell > naive b cell > plasma cells (secrete antibodies, undergo affinity maturation) + memory b cells (more immediate response to subsequent infections).

Anyone's welcome to correct me, I could be wrong, but this is what I remember.

Well I was asking in a general as someone with little understanding of immunology, but yes this would be an example. Let’s suppose there was an ethical way to study this response and it was found to be as slow as the initial infection, how many more studies would it take to discount long term immunity with a reasonable degree of certainty? I guess what I’m asking in general is how many different major pathways are there to immunization.

You're asking questions that make incorrect assumptions about how immune responses work, which is going to only lead to more confusion until you take the time to learn some immunology or take someone else's word for it. I recommend the former.
You're asking questions leading to more questions leading to more questions. You probably have a strong interest in immunology, you should go learn it you'll probably enjoy the content immensely.

I will not take the time to explain the concepts in immunology, virology, and cellular biology necessary to answer your questions, please understand I don't say this to be mean; you should pirate some textbooks and read them.

To answer bluntly:
>Response as slow as the initial response to infection
Secondary responses are always faster and stronger
>Discount long term immunity
The virus would need to mutate enough to escape your immune system's defenses which, in Coronaviridae, is a slow process (more so than the flu by orders upon orders of magnitude due to differences in virus family genome) or somehow infect you after your memory B cells have died (This is one reason people get booster shots, the other being hypermutation and affinity maturation)
>Major pathways to immunization
There is only one that I know of.

They can, and do. There are methods that certain psychometric tests can implement to detect deception. Certain individuals may be able to get past it but most probably won't. Casper is difficult to prepare for because you have a 5-minute timer and the questions are always unpredictable. But it generally gives adcoms a good idea of your character. Regardless, the issue is that there needs to be more scrutinization of applicant personality traits. I've known of people who set up fake organizations/websites/volunteer hours all to inflate their applications. It's incredibly fucked.

Dude we are not talking about average people here, more like two sigmas, probably smarter if they made it that far while being autistic or a psychopath. Especially when they know they'll be taking it beforehand. We once researched implementing a modified MMPI with over 2000 questions, but found out that people in the best 5%of IQ can reliably still cheat on it. If given a few sentences on what the optimal profile was the test became completely meaningless in filtering. This thinking that you can accurately and cheaply judge character lead into using these shitty measures currently in place.

You should just implement a standardized test that tests IQ, the ability to work under pressure and knowledge on the required prerequisite stuff. Rank purely based on that and let the school filter out the rest, the impulsive shitty people will implode in practice and as for the rest, well there is no way of reliably detecting them.

I don't think it's published anywhere, it was a part of project for creating a new selection process for a military academy (not the US).

I looked at CASPer and this seems like a thing that would be pretty easy to cheat, especially if you were to prep for it. Again, dumb and/or impulsive people wouldn't make it, but they can be filtered by pretty much anything. Also this murky subjective review stuff wouldn't fly here, especially if the test is used to do more than just screen people i.e. evaluate people who passed in relation to each other. This use would also pose some other problems; people's personalities tend to evolve for pretty long and stuff like social skills get better with practice, there is really no single optimal combination of personality traits for being a doctor, because the types of jobs that you might end vary so much: someone will always look at slides or images alone etc, and finally this really can't simulate the kinds of stresses people will actually encounter: plenty of people who seem to have perfect people skills crack and become assholes when they have stayed awake for a day or two, while awkward dude might just stay as an awkward dude. Thus selecting for cognitive abilities and diligence, and then filtering out people who absolutely are not suitable in school or in residency programs seems like the best way to me, as more thorough methods are prohibitively expensive and time consuming.

I don't see anything wrong with psychopaths who actually play out the role of a decent person for their entire life and I don't really trust our ability to pick them out with imaging in general. Actions are judgeable, what happens inside someones head is their business.

Exactly. Technology in medicine is on the rise but it's only there to aid us. Not from US, but when our hospital received its first DaVinci surgical robot, patients were very uncomfortable and thought that the robot would do the surgery on its own lmao. Same with radiology, just because you use a lot of technology, a computer may not see what you see or misdiagnose. We'll be long dead before any of that shit happens.

i don’t think anyone here would be, since you need many years of diverse clinical experience for that, and threads population is mostly students or residents. If you are really thinking about it you need to realize that your practice will be very different from what you are used to in where you live. During mission you will simply not have most of the equipment that you are used to working with and there is always big shortage of medicine and ppi on those places so you just work with what you have. Most importantly you don’t go there as an anesthesiologist but as doctor since you might have to do virtually anything from any specialisation so you have to have good general lnowledge of medicine and good social skills since you’re gonna have to tame and lead lots of niggers, even doctors among them are sometimes retarded and stubborn. Alas it would be a great thing for experience if you come back alive and healthy and it can be viewed as adventure too.

Radiology resident here.

AI is a minimum of 20 years from being used in rads. Look at mammography, we have had CAD for over a decade now and it fucking sucks. If we trusted it, 75+% of women would end up with unnecessary biopsies. We haven't even started using AI for other parts of rads yet.

Anyone going into radiology today will be safe.

IM is already a mental masturbation spec. You have to micromanage everything, little by little, check on blood panels every single day and keep everything in check. Now imagine doing that to an old patient that's riddled with comorbidities and it's unstable. It's a huge headache and considering they're on the IM ward, they're really sick. If you like it, sure, go for it, but it's too much of a hassle having 10-15 patients for yourself with multiple pathology, most likely poorly controlled, every single day.

Anyone here knowledgeable about nutrition?
I want to purge myself from all the nasty shit I've been taking into my body during my teenage years by eating garbage. I figured I'd try only eating stuff that hadn't been tampered with/transformed/processed, and cut down on the meat, so just:
>fruits
>vegetables
>eggs
>rice, beans, lentils, potatoes
>water
>occasionally some fish like tuna or salmon
Is this enough for a balanced diet or do I need to add some other shit? I basically want to eat as simply and healthily as possible and remove as many unhealthy foods as possible.

>> No.11930278

>>11930232
Remove sweets (especially ice cream) and soda.

Fruits (dried or not) + nuts (almonds/cashew) as snacks
Don't forget dairy products, you need a good source of protein besides meat. Chicken, turkey, fish are pretty nice, you can also have days in which you eat beef or pork but try to make it once a week at least. The rest is okay. I assume you don't have a goal like losing weights or working out to get ripped or something so this much should be enough to start your diet. You can make avocado spread and eat it with black bread, spring onions on top and tomatoes + cheese on the side for breakfast or idk, there are plenty of healthy food ideas around. You can also start counting your calories but as I said, if you don't have a goal besides "cleansing", then it's not worth the hassle.

Is that really ok? I read that adults don't have the digestive enzymes to properly break down lactose, and stuff like butter and cheese is always processed anyway
Regarding meat, isn't red meat carcinogenic? I thought eggs, lentils and nuts could cover the protein aspect well enough.

We can't break down a huge amount of lactose so it's different if you drink a glass of milk vs a slice of cheese.

>isn't red meat carcinogenic?
There is a risk to develop cancer at a later age due to red meat consumption, but IIRC it's only 0.3%, so it's very low, especially if you eat it rarely like once a week.
Eggs have enough protein, but lentils are more fibre and nuts are more healthy fat than protein but you can't rely on eggs only, I'd go mad if I ate eggs every single day. A proper diet is diverse. Japs for example have one of the highest rates of stomach cancer. Why? Because they eat shit. Miso soup, raw fish, pickled ginger with almost anything, it's just a trash diet for their stomachs. Try to be as diverse as possible and don't neglect any kind of food. Of course there's risk of developing cancer and food is not the only factor incriminated. Black bread is actually good, or you can make indian naan bread at home which is also extremely easy.

This is 100% accurate. Story time:

Mom had a friend during medical school which chose neurosurgery just because of the status. He wasn't that good of a student and he wanted some other specialty but when he saw his grade in the residency exam and how high it was, he chose neurosurgery. I'm not sure if it was on impulse or that grade blinded him so much that he fell for the "prestige" meme, but he ended up doing no surgery, didn't like it one bit and now he's practicing as a physician without a specialty which is called (in my country) "Limited competence physician" (roughly translated and worked for my mom until he landed a job in a small hospital. The role is equal to an american PA, he needs physician supervision to be able to practice.

Fuck status, and don't let the grades blind you.

>Limited competence physician
Seems like a stupid system. Here you can do most things as just an MD and you apply to residency programs when you want to. You can naturally also drop out or switch programs without any penalty.

You can apply to residency here whenever you want to, too. But I guess he just wanted a job asap.

I thought american MDs or DOs without residency are called associate physicians and still work like PAs.

Plus, the story happened in the 90s, idk how limited the places were or what was the penalty of leaving residency, but even if he didn't do anything, the resident salary of a neurosurgeon those days and even these days was/is probably the highest among surgical specs.

No clue.

If you can apply whenever you want, why didn't he just go to another program?

I'm not premed. I'm a humanities student but am interested in pursuing medical school after undegrad. I hate premeds just as much as you. If I am "technically" pre med in your eyes because I am thinking about going to medical school then I hate myself.
