Any dermfags here? I guess not but it doesn't hurt to ask
How bad is it really to shampoo every single day? I tried tapering off shampoo but dandruff and itching comes back every time. I don't know if it's seborrheic dermatitis or whatever but the only time when I don't have dandruff is when I shampoo every day with something like head&shoulders shampoo. I have long hair which makes it even worse. Any advice?

>> No.11907075

Do you wash your hair properly? Like massaging your scalp with the shampoo for at least 1 minute before rinsing? I had the same issue as yours, very bad itching and dandruff. Bought an anti-dandruff shampoo and massaged my scalp for like 2-3 minutes.

I also wash my hair every single day, but instead of a long hair, I'm almost bald lol.

You can also massage it on and leave it for five minutes or so (or until the end of your shower). Anti-dandruff shampoo worked much better for me when I started doing that.

Dentist/Orthodontics people:
I (32 year old, male) was given a choice between orthodontic camouflage and an orthgnathic surgery. I have no trouble chewing or closing my mouth etc. but I do have a lower jaw where teeth are pretty much aligned with those on my upper jaw (upper and lower teeth touch each other with no overlap).

The orthodontist was kind of clear that the surgery would require me to wear braces for a year before the surgery and that WOULD make me look worse than I do now. He was also clear that camouflage solves the problem with my teeth (and teeth only).

Three questions:

1. where do I get some articles regarding both options in adult people? from what I see males have it slightly better since a prominent jaw is a bit less of a big deal than in women.
2. can camoflage fix facial assymetry or push my lower jaw back a bit?
3. is it possible to do surgery before wearing braces or earlier than before the lower jaw is fully transformed?

Read directly the conclusion. Fine for neck, shoulder and anything that a PT can do 100 times better without cracking someone's bones but there is either lack of research or there is research of very low quality (basically shit, abysmal research) that proves that chiropractic "maneuvers" do any good to people with asthma, ileus and other conditions.

Instead of chiropractic, go to PT school. More useful, actually evidence based medicine and I think they're pretty based.

For me, the ranking goes MD > PT >>> Nurse >>>>>>>>>>>>>>>>>>>>>>>>>>>>> NP >>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>> POWERGAP >>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>>> PA (just because the PA copeposter is a piece of shit)

Depends for the surgery. No chiropractor will solve an appendicitis or something visceral like a surgeon will.

And the video feels biased as fuck. Just look at that first fucking comment holy lmao. I can still comment on it. If it's true, then surgeons turned him away because operation is too risky or the guy didn't have money to pay for the surgery and imo, the chiro took a huge risk. There are also plenty of cases in which they fuck up.

The psychiatry one is bullshit.

Psychiatrists in general have a disdain for psychoanalysis - the only reason we'd care about one's past trauma is a side note in our heads about a worse prognosis and a stronger recommendation for therapy. We're all just about pills and giving people advice on better sleep/diet/excercise if you're lucky.

Having said that I DO sometimes feel like a charlatan practicing medicine going completely off subjective data/feedback. I'd love to just bully a diabetic patient with their thicc set of labs in my hands.

Is sciactia a temporary thing? I woke up yesterday with pain exactly where the sciatic nerve travels. Def not muscle pain as I've had muscle pain in my lower left back.
It's very painful when I do the straight leg raise, when I cross my left leg and put my chin to my chest and when I walk. I did a couple stretches the day before but nothing that I feel could have damaged my back that bad.
I've done a few stretches today but im still having a lot of pain when I walk.

don't really want to just use this to diagnose me but I'm annoyed at how recurring this has become

occasionally my right shoulder will start hurting like a fucking bitch. I'm no medfag so the best way I can describe the location is like in the very center of the round muscle that makes up the outside of your shoulder. the pain feels like it's fairly deep inside and severely restricts motion, right now I can't even rest it comfortably in any position.

when this happens it feels better after a few days, but I can't figure out any cause. it's happened about once a month for the last few months, today I noticed it appear just after sitting at my desk for a few hours.

>> No.11912190

>>11911960
depends on the cause...
what is temporary anyway? Is anything temporary? sometimes a second feels like a thousand years... time is it real or an illusion? Is it linear or cyclical? if a were to say you have lived this same exact life over and over eternally and will continue to do so, what would your reaction be? if you relived the same sciatica pain over and over for infinity since conception(not since it is cyclical) of the universe can it really be temporary? or is it permanent in the most hellish sense of that word? Or will it be felt only once? Something like flow of river when it appears cyclical but no drop of water is like the other and never be the same. Then like the drops of rivers adding to each other did your choices and actions lead to this pain you feel? It seem like they certainly did, but as you chose what you wanted to do, did you also choose what you wanted? did you?

Here you have a pic of shoulder anatomy, then you have the tests below that pic but you'll need a physician/PT to help you with them. If they're positive, then you might have bursitis. As I said, it's unlikely but it's possible.

Here it really depends where you live
the closer to the sea you live, the better access you have
Public healthcare while very much lacking in resources compared to private clinics because third world is VERY cheap. You could get a consultation for like 3 dollars and the state's healthcare insurance company will refund you all of it still.
However since conditions are so bad, doctors tend to leave which ends up in insane waiting times and month long waits for appointments for some specialities such as orthopedic surgery.
Private Doctors however have pretty much no appointments, very low waiting times and surprisingly good quality of care sometimes. But the cost of care is higher at around 25$-30$. You should keep in mind most people don't make a dime over 330$ a month when you look at these figures.
Overall compared to the rest of africa the quality of care is really good which shows by the amount of medical tourists. But that ain't saying much really.

my bad, lost in translation
GP offices are very accessible if you are in the big 4 cities (Tunis, Sousse, Monastir and Sfax) with quite low wait times. Otherwise access to primary care is quite difficult. Some people will take hours of transportation just to see their doctor which ends up in people in rural areas letting conditions get the worst of them until it's too late
>medical tourists
people (usually from a subsahran african country or a neighbouring arab one) travelling here in the hope of seeking better care for their condition. We do it too in europe for operations that are not done here due to how rare they are or lack of training in that perticular procedure

But it's quite hard to explain why'd you'd wanna leave the city which you've been in for years in med school, just to go to the middle of nowhere to a community you barely know.

Yep, same shit at my uni. I've been born in the city and I'm kind of fed up with it. Lived in the suburbs for a few years then back to the city (this time on my own + fiancee) and the city is just too loud and crowded for my taste. I really like rural areas and have plenty of them around the city (ranging from 6 to 12km away from the city) and would like to work there instead. The city is also oversaturated with physicians. One more and what good can I do? At least there I can form different kinds of relationships with the community and become a part of it instead of being a slave to some old ass commie attending for the next 10-20 years after residency. Hospital life may be a bit more entertaining than a rural gp office but there are too many rules and too many asses to kiss and not to forget the competitiveness of your peers.

Hey anons. Second year medical student with strong interest in psychiatry. Am unsure if I could ever get into a sexual or romantic relationship; would that mean I couldn't be a good psychiatrist without that experience? I'll have to decide on what kind of residency I'll take in a couple years: a psych residency? Child psych? Family care (not super interested)? Pain management and rehab? (somewhat interested...but I really want to be a psychiatrist). There's also combined programs like family med/psych that I might do. IDK: if I haven't kissed anyone by the time I've got to decide, is a combined program a better choice for me? I'm 25, if that means anything.

You still have a long time to 1. Get laid and 2. Change the spec you want to. I heard many times peers that were dead set on a spec for two or three months and then change to something else. Finish with your clinicals first, you might like something better.

Very strange request. Its very apparent you are new to medicine since with most conditions you could never say with full confidence they were caused by factors the patient was in control of. Your saying its the lifelong smokers "fault" they have lung cancer even though many people develop lung cancer without touching a cigarette. You could never say it was the smoking produced the cancer however it is likely to have played a part.
>lack of self care even if they have been educated on how to change

Inform your patients in a way that they understand and really trust. This is really important in ensuring they adhere to the prescribed programme, medication or lifestyle changes.

>> No.11915551

Have you had peers that were dead set on a spec for ten years or more? I've wanted to be a psychiatrist since I was 14. I've heard from several trusted friends that I kind of needed sex and relationships in order to be any good as a shrink. My argument is that you need to personally experience something in order to understand it (or at least sex and relationships) and many of my patients' problems will be relationship-based.

I'm on the spectrum, too; while I can have friends and do OK on things like medical school interviews I think I'm just too awkward to ever have a relationship - I think that people pick up on and are viscerally disgusted by the awkwardness.

False. You're not doing sexology you dummy. You'll have schizophrenic or suicidal patients. You never experienced voices in your head or never truly wanted to kill yourself. What difference does it make if you had sex or not, lmao.

Not disagreeing with this but I'd argue a nuanced approach is required. For some patients, hearing that they caused their disease is likely to make them disengage, whereas for others it can serve as a wakeup call that they need to get their shit together.

>>11915232
Look, if you pointed a gun at my head and asked me this, I'd say anything to do with genetics or pediatrics is a start because they tend to get inherited diseases rather than lifestyle related problems. Psychiatry is arguable as well since patients tend to come from shitty social situations (which may or may not be due to their own fault).

But wait till you go through clinical like other anons have mentioned. Last years of medical school are times of rapid change in specialty interest for most

>argue a nuanced approach is required

I completely agree, in the Uk we call it the nudge approach. Often our aim is to just give a slight push in the right direction. Also timing can play a big part in getting someone to give up smoking for example. Say for two weeks someone stops smoking because they have a nasty chest infection and are on antibiotics. GP's will focus on these individuals to promote anti smoking messages since studies have shown these people are more likely to quit having made the first step themselves.

Any ER med anons here? There is a conference oxford school of emergency medicine. My uni wants me to be on a comitee or something to promote EM to my peers. Is stuff like this important for my future career Its online so I could probably just say I attended or whatever. Also would being on the committee be useful since applying looks like minimal effort but does it yield any clout later on.

autonomic nervous system is part of the peripheral nervous system that acts automatically without concious effort
for example on the smooth cardiac muscles and the glands (we do not control it)
it is divided into 2 parts that work together in tone as the balance shifts according to the body's needs
though some parts of our body are only affected by one, for example the smooth muscles of blood vessels are only acted on by sns fibers to keep normal blood pressure by keeping them constricted
sympathetic nervous system (fight or flight response)
it arises from the thorax-lumbar regions of spinal cord, the fibers exit until they reach a nearby ganglia (basically a collection of neurons in the pns), once they are in that chain of ganglia, then they may follow 3 routes, some synapse immedietly with post ganglionic neurons, others go up or down before they synapse, some of them cross the chain without synapsing and this group travels further to reach collateral ganglia where it synapses, then the fibers run all the way to target organs
it increases the things we need to make certain reactions like we're under danger and also inhibits the things we don't need
e.g. it increases heart rate, increase breathing rate, dilutes the pupils, realease glucose, inhibits digestion&peeing
the effect of sns is very widespread because high divergence (1 pre ganglionic fiber : 20 post ganglionic neurons)
parasympathetic nervous system (rest and digest)
it arises from the cranial and sacral parts of the spinal cord, that's why it's called para it surrounds the sns region and travel to the parasymepathetic ganglia which is near the targets (the post ganglionic fibers are relatively short in comparision)
act when the body is under rest. lowers heart rate, breathing rate, normal pupils, store glucose, and more digestion&peeing
very low divergence compared to sns (1:4) so it's more specific localized responses

What's the point of taking temperatures anymore?

I went to the barber shop today and I was talking to my barber and asked him why he didn't take my temperature. He simply replied by telling me that it's useless.

1. You can be positive and asymptomatic
2. If you have a fever, you won't be coming to the barber shop

I mean, he has a point, but I guess it only applies to such businesses and not supermarkets, etc.

I had that once for like a week and then developed conjunctivitis in that eye. It's probably inflamed or irritated by something.
If you've been having that for a long time though, it's a neuro issue probably.

After reading an article about an acupuncturist who gave his client a pneumothorax, it's not an option even for hypochondriacs anymore.

Just give them vitamin C instead. Everybody loves vitamin C. I love vitamin C very much.

I learned it the hard way.
>Hey anon, take this 120kg patient on a wheelchair to radiology
>Hey anon, take these samples up to the lab
>Hey anon, the patient in bed 3 needs some water, go buy some for him (from his own money)
>Hey anon, there's some blood on the floor, go clean it up (yes, this happened)

After that one night, I was exhausted and miserable. Next shift I ignored the nurses. Fuck them. ER nurses are a headache. Instead, my hot cougar attending saw that the nurses abused me and took me under her wing and showed me actual clinical stuff. I think I told you guys this in the early /med/ threads.

Makes me wonder how large acupuncture needles are
Are they like 26G? Thicker? Thinner?

>> No.11918647

>>11918641
>This is the one a lot of people get revved up about because they envision huge hypodermic needles, which have an average diameter of 0.01 inches. Acupuncture needles have an average diameter of 0.00325 inches. That’s like a strand of hair.

They also come in three sizes of length: 0.5, 1, and 1.5-inch needles. I use a 0.5-inch for ears, 1-inch for the face, hands and feet, and 1.5 for the leg and larger muscle groups.

Needles are measured in gauges, and the higher the gauge, the smaller the diameter. Hypodermic needles are typically between 25-27 gauges, and most acupuncture needles range between 30-40. The largest I use is a 32 gauge, and that’s for treating pain. Larger needles help break up stagnation and muscle spasms, but otherwise, thinner needles are the way to go.

In treating typical fertility conditions, I use thinner needles because I’m trying to influence energy – which doesn’t require big needles – instead of breaking up pain.

All I could find.

hm, I'm used to 26-27 for spinals, and didn't think needles as thin as 30-40 could cause (clinically significant) pneumothorax

>> No.11918701

The conclusion we came to is that its stress related, usually triggered as I'm working sometimes when I wake up. Loss of balance if I'm standing up, otherwise nothing else.

Honestly I was quite on my own, it was mainly because I was periodically anemic since a young age and when I felt a little down lately I decided to start taking iron supplements and only afterwards thought it would be a good idea to take an examination.

And no I'm not a woman but a man, for the root of my anemia they never really figured out, I think they just said it was genetic since my mother as well as some other member of my family were often short on iron.

Well there's a theory that certain headaches can be caused by contraction of scalp muscles or they're related to those muscles in some way. Phoneposting now and about to sleep so I can't lookup the details. Headache location is definitely not strictly correlated with the underlying pathology (sometimes it is but the majority of time not). It's good that your MRI scan didn't show any masses, that rules out the insidious option. It might be stress related as your doc suggests. Maybe a weird variant of a migraine? (migraines are like the lupus of headaches, they're always a suspect whenever you have a weird headache that doesn't fit into any category precisely)
