It all derives from traits being greatly polygenic, which means all traits are impacted by many genes. You dont have like, just a gene that makes you tall. Theres a gene, and it has a slight impact on your height, and your liver function, and your sleep cycle, and every other trait.

What this means is that traits generally come together, and that genes evolve to be good at many things.

So, the end result of this, is that people who have crappy diabetes genes, generally also have crappy hypertension genes. They generally have both because generally the same genes are responsible for both problems.

But this extends to every trait. So people who had crappy hearts also have crappy metabolisms, and crappy attention spans, etc.

Dumb people who dont make very much money are also generally unhealthy, because dumbness and disease stem from the same genes.

To answer your question, OP, its a combination of a few factors, (1) poor people are stupid and don't care as much about things like health, education, art, etc.
(2) eating healthy, working out, spending time outdoors, etc. is expensive.
(3) poor people tend to be lazier and are less willing to spend time cooking healthy food or exercising. They would rather watch TV or use social media.

I think it's because

A. If someone works on a job that pays poorly, their food budget fluctuates on all the other needs you have (utilities, transportation, children if they have any, etc), so in general they want food that gives them more bang for your buck
B. If someone works minimum wage, they are often working on some front line job that's more physically demanding job than someone working a purely administrative job so after a long day, therefore fast food is seen as a quick cheap option than to cook a 40 minute meal after returning home from work.

TL;DR: Fast food is seen as cheap and convenient for someone earning minimum wage, therefore leads to a poor diet which causes diabetes and hypertension

BUT, the variation in diabetes is not explained by carb intake alone. Most of the variation is genetic in origin. High sugar diets will cause diabetes for white people and native American people, but the sensitivity native Americans have is much much higher, and indeed most of the difference in diabetes comes down to ancestry, rather than diet.

Just showing that when diets changed diabetes incidence changed is not enough to rule out genes, because the same diet change doesnt necessarily have the same change in diabetes incidence for another group.
