Have a meeting with my Master's advisor tomorrow (engineering). I canceled our meeting last week and in the meeting three weeks before that, I told him I'd done absolutely nothing in the previous three week window. I've done absolutely no work this time either. Do I tell him the truth about how I just don't feel like working on my thesis anymore? Should I lie and tell him my grandma died?

If you're still collecting data / designing experiments, you can easily think of issues that you might run into and get his input on how to deal with them

You should frame it a different way. Convince him that you think you need to go speak to a counselor and that there must be something wrong with you. Depression is common in students, and you never actually have to talk to a counselor because odds are they won't have time for you.

I'm back. He told me it was OK if I needed a break and to take some guilt free time off if I need to. Said I've done some good work so far and that I'm near the finish line. He's a nice man. I feel pathetic now though, like a failure. I don't know why I do this to myself.
