Hello med fags:
I am a dentist . Spend way too much time in hospital for autistic and retarded kids and make no money doing it.

Here’s my question: all the physicians and nurses appear to condescend me for being a dentist. That’s fine, they’re friendly but you can just tell they think they’re better.
My question is: is it real or in my head?
Respond your view and your occupation. Ie: dentists are retarded I’m a nurse

Do I have a herniated disc? For 3 weeks I've had pain in my lower back after deadlifting badly. The pain only happens when I start slouching i.e. bend my lower back a lot. The pain sometimes also feels more like soreness, it really doesn't hurt that much.

is it dumb to try and diagnose my neurodegenerative condition?

I am 21 years old and have heat intolerance, shortness of breath, memory loss, trouble focusing in college, inability to work or drive, and also erectile dysfunction.

the dementia progresses rapidly because only 2 months ago I was in college studying and now I cannot focus on TV, video games books, or reading.

The neurologist said possible testing includes
-imaging besides MRI(already completed) like PET scans
-exome sequancing to check for genetic diseases
-IQ testing (IQ was over 130, 2 years ago)
-lumbar puncture to check for neurological disease

Honestly if it's legitimately neurodegenerative you're probably completely fucked and diagnostic tests are likely going to be expensive if they're not covered. On the other hand there's a chance you're not and could be treated. If the results come back positive you would be able to at least try to reach out for studies that could eventually be beneficial to other people down the line. There isn't really an objective answer.

Are you sure it's a bacterial infection? Pharyngitis is usually viral and has some clear signs when it's bacterial. Ospamox is a shitty choice anyways, Amoxi+Clav is the superior choice for throat infections. If you can look into your throat, looks for fire red delimitation, and pus like vesicles on your tonsils. If you find it only red and spread all over, it's usually viral and you should quit taking ospamox. It's not the patient's choice, radiation sessions are usually once every day for 30 days because you don't want to induce metastasis from too much radiation. The way onc goes is like this:

3 big options - Surgery, Chemo, Radiation. There are tumors you can't operate on/too late to operate on, chemo and rads are double edge swords since sometimes they can do more harm than good and radiation because as said earlier, it can make new tumors appear. So, switching to chemo after radiation therapy is pretty standard.

Not your fault, you did not know as I don't really either, I don't know all the various methods that exists for cancer treatment, for example there was my grandma who had breast cancer and my mother told me her treatment used some pills she did not know so I guess that was outside of radio/chemo too.
>Immunotherapy
I would not call it that, because they certainly did not, but is something similar, in the sense that it was slow and about stimulating the immunity system.

So, a while ago I went to the dentist to get a bunch of fillings. After I had a couple done, I got an abscess and he put me on antibiotics before fixing that tooth as well. Now the area is red and swollen, and I get a weird taste in my mouth whenever I touch it with my tongue, but the other fillings are fine. Did he do a shit job treating the abscess or what? Should I pay him a visit?

I have a question about teeth clenching that's been bothering me and was wondering if anyone could help me with it.

Around and since my college graduation about 5 months ago I've noticed certain nights I wake up and it feels like I've been clenching my teeth. I think this is due to stress/anxiety, but I'm also concerned about whether or not I've always been doing this and am only just noticing it now.

If I did have a clenching/bruxism problem, would/should my dentist have been able to tell/check for it during all my past dentist visits with him? I've had tooth bonding on a front tooth, but I'm pretty sure that was from a bad habit of biting on pens during high school (and he told me to not do put any foreign objects in my mouth when I pointed out the mysterious chip in my tooth). I have never had any cavities, noteworthy tooth sensitivities, or enamel problems as far as I know. Never have had jaw or joint pain, or headaches either. I can't tell if I've ever had attrition like tooth chipping/flattening anywhere.

I hope this is just a recent problem stress induced clenching problem and I can mitigate it with stress relief.

Hopefully someone any ideas. Appreciate it, thanks.

I am a dentist. Generally I am against giving personal advice online. However next time you are in ask for a night guard, it should be deisgned to place your mandible properly to relieve pressure on the TMJ and absorb the force rather than you grinding your teeth. I personally wear one l. They’re expansive but insurance usually covers one every 5 years or so.

Also, if you don’t when you’re 70 or so you could grind down to the nerves and need very expansive root canals and crowns to rebuild the whole occlusion. Far cheaper to get the night guard.

Unless you can work for a municipality (id est: Fire Department), avoid this industry like the plague. Private EMS is the worst fucking thing you can do. The units are garbage and the stations, provided you actually have one and don't post on the side of the road, are worse. My station just had a bedbug infestation. My service has two owners. One is a rampant narcissist and the other is about as easy to contact as fucking Bigfoot. I once got suspended because a guy rolled a unit over on my day off and somebody told admin that I drove to fast once. It is an arbitrary, capricious, shitty fucking industry.

If you go into nursing, go straight to RN or higher and pack on as many certifications as humanly possible. LVNs get stuck working in nursing homes and home health services giving meds to trailer trash. RNs with no additional certs end up in MedSurg. Normally that wouldn't be so bad. But MedSurg is where hospitals stuff their diversity hires. So if you're white you probably won't even be considered and if you do get in, you'll get stuck doing all the work for a bunch of negresses. Also, if you're male you will be a constant target for every virulent strain of female bullshit you can imagine and a few you can't. To put it another way: My workplace is toxic as fuck and I won't even consider nursing as an option.

This. All of this.

Plus: Nurses are cunts and will call in and complain about you for anything. Some examples with the actual event in plain text and the complaint in green:

A nurse at Texas Children's in Houston wanted me to do her vitals on a patient I transferred there. So she literally snapped her fingers at me and pointed at the dynamap. I snapped my fingers back at her and hooked my thumb at the thing, then turned my back.

> Your paramedic was rude and refused to give me en route vitals on a patient. He was also rude to the patient and her family.

I was working a code (CPR) in progress at a local chicken processing plant. We loaded the patient onto a backboard and started to lift him when the board slipped and the patient's entire 300lb weight landed on my thumb. I said "Fuck!".

> Your paramedic was rude to our staff and shouting profanities at them. (got fired for this one)

Had a conversation with my partner in an elevator in a hospital 79 miles from my station regarding a coworker of ours who fell asleep in the back of the unit.

> Your paramedic was bragging about how much he slept in the ambulance with patients loaded.

Brought in a code to a local ER with only myself and my partner in attendance. When we got to the ER, the charge nurse came out to the bay and stood there watching us move the patient into the ER and openly asked why I wasn't ventilating the patient (note: I was doing compressions and operating the stretcher and she literally stood there and complained without moving towards the stretcher).

> Your paramedic refused to ventilate the patient during a CPR in progress and I have sent the report to the hospital administration for review.

Overall, I like the work. The problem is that I got in a long time ago and ended up stuck in this area because of my kids. There are two municipalities I could work for, but they pay for dogshit and are run by exactly the same kind of despicable cocksucker as every other service around here. So I drive two hours one way to work 48 and 72 hours for an interfacility transfer service. Which actually isn't that bad... on the face of things. The pay used to be good (they never kept up with local trends so they're actually underpaid. Their insurance is the tits. Their units and station used to be good but the owners have become dysfunctional as fuck and never deal with anything anymore. And, nursing bullshit aside, the calls are usually gravy. I did that 911 bullshit for two decades. I'm fucking done with it. I just want a nice, cozy place to curl up and make my overtime without worrying about some crackhead trying to stab me (it happened).

And things are better now than they were 23 years ago. The nurses being cunts thing is just the way a female-dominated industry is going to be. My present employers get this and usually ignore complaints. But there are still huge problems that will never be fixed and that's just how it is.

Still, I'd rather jam a red hot poker up my own ass than be a nurse. Fuuuuuuuck that shit. Those cunts are fucking horrible.

I had a relatively difficult childhood and I went throught what I hope was the worst part of my life between age 16-20.

I did a lot of mistakes, learned from them and I now understand that only myself can sort me out.

It was never a dream for me to become a doctor as a child, but for a year now i've come to the conclusion that it might be the best course of action for me in order to find happiness some day.

Med schools in my country are free and entrance is an open competition exam.

Thing is I think i'm kinda set back due to the fact that I damaged my brain and body quite heavily during the last 7 years.

But I still think I can do it. I quited smocking weed and almost tobbaco, fixed my diet, stopped strocking my dick 3 times a day, started reading again and several other stuff.

But i think it's not enough so this year, I'll start to exercise and re-learn fundamental science in order to be ready next year.

Any advices ? Do you guys think I'm being delusional or courageous ?

Also thanks to whoever posted the "learn how to learn" online course on coursera in a previous thread, that helped a lot.

Breast cancer is easy to treat if detected early (just like most types of cancer). If detected early, surgery is best followed by some radiation and chemo sessions to ensure it stays tumor free. It's better to remove the other breast too because there is a slight chance it will get malignant. (Slim, but it's still there, I'd rather have 0 chances of getting breast cancer than any chance).

I don't know what you mean by that question, what type of tumor? I honestly have no idea, depends on the stage you get it detected, the efficacy of chemo and radiation, patient compliance, it's not that easy to treat cancer, certainly not the same as a disease you usually see on the internal medicine ward.

It's always different from patient to patient. My dad (RIP) was 47 when he was diagnosed with Hodgkin's lymphoma, detected early, docs gave him 80% chances of survival at 5 years but the tumor didn't respond to anything, although it was sensible to radiation. In 2 years he was dead.

The problem with cancer is that the signs are non specific and it can manifest in a lot of ways leading the patient to think that they just have a cough, a persistent one but not something serious and it turns out they're full of metastasis.

I assume that injection was not only a one time thing and your fiancee was in palliative care at that moment, maybe something to soothe her pain and make her end of life a bit better. Multiple organ failure is usually the cause of death in cancer patients, a pretty shitty way to go out because it's a fucking torture, the only way to improve quality of life is to give the patients medication such as morphine, noradrenaline and such.

The lumbar region is not designed to flex much, with forward (fold at waist) bends being particularly risky. At the same time there are lots of nerves running through the spine and sacrum. Basically, stop slouching. To get the musculature back in balance, stop squats and deadlifts, and stretch legs, quads, glutes, etc. for 2-4 weeks until things are loose again and then you can continue with focus on abdominal core work for 2-4 weeks and then maybe squat again with laser focus on perfect form, but don't rush into it. Its imperative to strengthen the core to support the spine, this is where people fuck up because core is boring and strenuous and they think they can skip it.

If it is a herniated disc, increasing the intervertebral distance and relieving pressure will help healing process. That means unloading the spine, stretching, massage

To ease the pain lie on back, bring knees to chest to flatten the lumbar region.

The sheer level of competition is insane. Everybody in my classes was the smartest kid in their high school. Everybody is at least as much if a science nerd as I am, and they're better at it. Everybody has their shit together, and everybody is assumed to have their shit together. No mercy, right out the gate. Weed-out courses. Class averages on exams are C's and D's. Tons of homework and no hand-holding.
Every class is a math class. Every class assumes competency in so many other areas. There are 1100 of you and they'll totally fail all of you. You're just a statistic. Being smart ain't shit. B students are average fuckups in here, and you're gonna study your ass of for that B. You're gonna bomb an exam and spend the rest of the semester clawing your average back towards the mid 80's. Do the math in your head constantly and realize that you're gonna have to suck the professor's dick for a grade in the low 90's
You're just average here, and average sucks. C's are practically F's, and you will be repeating that course if you get a C. Your TA is disappointed in you but also he doesn't care.
You will not sleep. You will learn to manage your time. You will go to office hours and get tutored personally, you will develop an addiction to caffeine or some other stimulant, and you will pay in days for hours you dicked off. No weekends. Check your email constantly and neurotically. Develop an anxiety disorder and then learn to achieve nirvana through constant exertion and dread.
You will be humbled. Any pretenses of being able to do any of this shit without trying will wear off immediately. You will fuck up somehow, and that will somehow come out of your loans. You will take out a lot of loans. You will study and study for the easy classes. You will take classes this summer. You will do internships and extracurricular, and you won't even be able to find time to do them until you're good enough at this shit to be getting A's all the time.
You will...

The only deal with NPs is that due to the lower training time than physicians, you have to specialize more. Adult only or Ped only NPs, Acute vs. Primary Care, etc. Granted there are some lifespan NPs (FNP/PMHNP/ENP).

Excellent post. FNPs recognize the generalist role in the community whereas specialization certification affords more inpatient opportunities. No different than residency assignments and ultimately I do think the barrier to entry will be DNP although presently much of the post-master's level education is in research and educational theory utterly useless in clinical practice.

Pretty much this. The only way midlevels can help physicians by practicing independently is to go rural.

Romanon here. This is aimed at US students. Why do you guys hate midlevels?

From what I've searched on Google, yes, they do have a fraction of a training of a doctor, but you don't need to go to medschool to treat pharyngitis, a NP can do it just as good.

Speaking about them practicing independently, it's kind of a double edged sword from my POV. It's okay to let them do it to treat minor stuff and monitor patients with chronic diseases, but when it comes to diseases that manifest in a non-specific way such as cancer in early stages or the debut of a chronic disease and they just shake it off, it's kind of a disaster.

All in all, I wish my shithole had these roles to help out physicians, but I don't understand the hatred towards them. What makes you despise them as much as the internet shows? I've seen some heated discussions on different forums and they just bash the midlevels like there's no tomorrow.

Hi medfags, 2nd year undergrad student here.
Have some pain in levator scapulae worsened by extending arms against resistance, been there for about 3 weeks now, muscle tone is noticeably stiff and there's some slight winging.
Physiotherapy is a 4 month waiting list... Anything I can do to work out the spasm in the meantime? Currently just taking NSAIDs and trying to correct my posture.
Thanks.
