We discuss research, offer advice (the best one is to see your family physician), make fun of premeds and shitpost.

Do not engage the clamp poster (https://pastebin.com/aYNv0zFc).).

Don't start discussing pro- and anti-vax shit because it degenerates and takes a lot of replies.

What are my chances of getting copd ? I am 19 , do sports, overweight. Three years ago I was in my primary care and doctor told me I might have asthma for me to get some 2 other medicines I had to be hospitalized (probably long acting corticosteroids) but I can get salbutamol right away. Pick it up use it 3 times, feeling better, throw it away.

2 years ago I was feeling like shit coughing, chest pain and other. Told a friend to buy me ventolin he brought it, got better use it frequently till this day.

6 months ago I got terrible chest pain, walking was so hard and shit, salbutamol was of no help, went to er, doc said it's airway obstruction, sent my father to buy some stuff from pharmacy, including antibiotics and antihistaminics plus something for i.v. I refused antibiotics went with others, felt immediate relief. To this day I still use ventolin few times a day on most of days (some I don't). Never smoked, but people around me do smoke, air is poluted af, especially during the winter.

"Cure" research for autism is focused on identifying genetic markers. That research will plug along unhindered. Once we've found those genes, we will be able to tell in-utero whether a given embryo is autistic. That means that the "cure" will simply be abortion. You can argue that we'll use CRISPR and gene therapy to fix people with ASD, but that's not realistic. Gene splicing doesn't work that way, and even if it did, abortion is easier and we already have the technology for it. And with the inevitable overlap of genes in high-functioning and low-functioning autists, it will be better to err on the side of caution.
When people are given the option, they will choose to abort autistic babies just like they do with down's syndrome. And, done early-on under the guise of a right to choose, that decision will have the justification that simply abandoning or murdering that same child outside of the womb would lack. They will be erased long before they ever have a voice.
Arguments for the subtle differences in individuals and for the spectrum of manifestations will fall on deaf ears. And the numbers will back up the parents; you're far more likely to end up with a shit-smearing, tortured retard than you are with a quirky savant. It will be seen as merciful and kind not to burden such individuals with the breath of life, and as always, people will be quick to speak on their behalf and assuage those pesky moral doubts. They will assure us all that their lives were never worth living to begin with.
Society will never have to deal with autism again. In the end, those who the "cure" was always intended to benefit-the parents who are burdened by having to raise autistic children-will get what they always wanted out of that "cure": the chance to raise somebody else altogether instead.

>> No.11007958

>>11007954
The same parents who imagine that their autistic child is simply a normal person trapped in a defective body will jump at the possibility. The ones who daydream about somehow, one day, removing that aspect of their children and leaving behind a normal person possessed of all the things they love about them-they'll get what they were always really after.
That way of thinking-gone. That mode of feeling and perceiving the world-gone. Those interests and that way of fixating on them-gone. That flap of the arms snd clapping of the hands as they jump up and down in joy and excitement-gone. The way they bounce on their tiptoes as they walk, the odd cadence of their voices, those long-winded rants and those strange rituals of theirs-gone. We won't be removing some secondary aspect of those people and leaving intact souls behind-we'll be erasing everything they ever were or will be and discarding it as medical waste. Which, really, is all we wanted to begin with.
God forgive us.

In medicine there are equivalents of doing what you do, although not to the extent perhaps that you’re accustomed to.
For example, it’s very easy to coast through life as a junior doctor, outsourcing all important clinical decisions to senior staff and never worrying about exams or research to progress through a specialty. However I can’t imagine anyone with an ounce of self respect wanting to take orders or defer to someone 10 years younger than them for no other reason than their own laziness and apathy.

Synaptogenesis has a solid scientific background on the basis of the nmda receptor pharmacology. Contrary to expectations, excitotoxicity (eccessive glutamatergic activity) doesn't increase by any mean your brain functionality but is a key feature in neurodegenerative diseases (to the point that antiglutamatergic drugs have label indications such as riluzole for als).

The idea that taking some medications increase neurogenesis is just silly.

I can't figure out wtf is wrong with me.

S/S: 6/10 abdominal pain radiating to back with discomfort level of 9/10 an hour after consuming Tuna, Tilapia, Portobello Mushroom, Wood Ear Mushroom, and egg. Pain comes and goes every 15 minutes and lasts for roughly 6 hours. Tuna "intolerance" developed last March, then mushrooms in September, and eggs very recently. Smaller concentrations are ok, such as bonito flakes, mushroom extract, or egg in pastries. After an "attack," dull lower left quadrant pain can be felt at night as I pass gas.

Blood test only came back significant for homogeneous ANA pattern. I have a persistent cough, dry heaving issue only when it's cold for 6 years now, and the only drugs I use is modafinil recreationally 2-3 times a week. Caffeine 1 cup of coffee a day, non-smoker and drink occasionally.

What the absolute fuck moment today at peds.

Go in with two colleagues (I'm the only guy in my whole group), 4y/o, no rhales, pharynx is red, tonsils are swollen, red and have white pus in the crypts. To make a differential, I wanted to consult the kid and the kid started to cry when his mother lifted his pajama top. One of my colleagues told me to stop because I was traumatising the kid...???? What in the fuck? How the fuck am I supposed to make a physical or even a crappy differential diagnosis without even touching the kid?

Took both of them out and I fucking snapped. I'm usually not this short tempered but this time, I don't know why, I just snapped and told them both to fuck off and let me do a physical and they went to the Prof, kek. I got a little lecture about how to talk with my peers in the future but other than that it was fine.

jesus fucking christ... been there too. Personally I hate peds, I have little patience with kids desu.
But when you have to make a physical exam you just have too, no two ways about it.

btw when they cry is when pulmonary auscultation is at it's greatest, despite what one would think otherwise! On recurrent wheezers some times you don't even need your stethoscope to hear the wheezing when they start crying.
Your colleagues should know better.

I'll try help you just for fun tho...
Any abdominal ultrasound of the right upper quadrant? or any abdominal imaging study for that matter?
How old are you? does it happens with fatty foods? any allergies on your family or you? any changes on your stool? have you lost wight lately? any relevant diseases in your family? what lab studies have you done so far?

So far, according with your description this could be anything, the first thing that pops to mind is chronic cholecystitis (quite rare tho, depending on where you live or your ethnicity). But it could be a shit ton of other things, from food allergy to a median arcuate ligament syndrome. It could be anything with your description, you gotta be more specific.

I've practiced on kids before, I know that auscultation is at its peak when they cry but this kid was more like half crying? I don't know, it seemed forced to me, I did consult him anyways, I couldn't care less if he was crying or not, I really needed to finish the consult properly. My colleagues, I think, are too sensitive about it and try to do everything with as little noise as possible thinking that if the child.cries, something must be wrong. Fuck them anyways, tomorrow I have peds again and can't wait to see what patient are they assigned this time.

Ate some shellfish in a restaurant yesterday and I'm trying to figure out exactly what may have caused the food poisoning which followed around 6-7 hours later. Some dip with fries was served as well, and I drank some water from a glass container. Didn't eat anything after.

I'm around 28 years old.
Symptoms were milder than what most sites seem to suggest from shellfish:
>nausea
>followed by emptying my stomach by vomiting (I had some control and more or less decided vomiting would be better, but I didn't exactly induce it)
>there was also very little diarrhea (less than what you'd expect)

The whole ordeal was more or less over in about 40 minutes, most of it trying to ignore the nausea in bed.

Haven't had any of the dangerous symptoms like prickling of skin, convulsions, confusion and short-term memory loss, problem with speech, blood in vomit/stool (what little there was).

I was fairly cold when I decided to go to the bathroom and sort of shivered while preparing myself for the possibility of vomiting. Although I haven't and didn't really notice any particular temperature sensitivity: likely it only followed with the nausea.

Stomach has recovered and I've eaten without issue, but my stomach is still a little sore probably from the vomiting.

I don't have anything to worry about, right? Was it really the shellfish? Haven't eaten anything supposedly risky the last months.

I'm from Denmark btw.

When I'm nervous, something in my nose tenses up and it gets harder to breathe. It's especially frustrating when I'm trying to fall asleep.
What is this fenomenon?

Just to make it clearer - I don't start breathing heavier, it's more like if my nostrils contracted.
Also, when lying down, it's worse on the side than on the back with the nose facing up.
Blowing the nose makes it worse.

>> No.11020934

You got just an acute gastroenteritis, probably due to food poisoning, basically you ate toxins produced by bacteria. You should be fine.

Your biggest concern should be dehydration so keep drinking isotonic liquids (energy drinks have way too much glucose so you will dehydrate further so try avoid any sugary drinks). Look it up on google how to prepare your own isotonic drinks, its quite easy.

Be worried if you start bleeding when taking a dump, if you get very dehydrated, if your symptoms get worst or they persist over a week, if you develop fever, if you develop respiratory symptoms like if getting a cold. These symptoms will indicate that you got a bacterial infection that need antibiotic treatment and assessment by a physician.

Also, we can't tell if you got sick due to the shellfish you ate but it is most likely.

Like a nose twitch? probably just a benign fasciculation.
Benign fasciculations, muscle twitching that is, can be secondary to anxiety, dietary deficiencies, lack of sleep, medication, you name it! annoying but generally not dangerous at all, we all have those.
They can be repeated moves (twitching) or just one sustained spasm. It shouldn't last more than 3 weeks tho. If it lasts more than that then I would recommend consulting your physician.

Back in med school I used to get eyelid fasciculations every time we got into the exams period just from anxiety. It was annoying as fuck.
(wrongly called myokymia, but only if you want to get very annoying with someone since technically the only difference with a benign fasciculation is seen in electromyographic studies)

Complex question: I'm nosy and I just want to find out: I went on clinical rotations giving flu shots for school and the pharmacist leading the event offered me and my classmate free flu shots. We filled out the paperwork and everything. Do pharmacies just give out flu shots like that? Or is he getting reimbursed somehow? He partnered with the company we were doing the flu shots at...So I was thinking he gave them to us so the company will reimburse him because we fall under some sort of special category. Or maybe he gets reimbursed because he considers us employees of HIS pharmacy...idk, guys just help me find some dirt on this.

They have this image that every student who comes in is hot shit who's going to tell them how to do their job because they naturally feel inferior and threatened to newer or 'higher up' staff (nurses vs doctors etc.). Nope. They even mention in the show in the first season that his billing is basically nonexistent and his whole department is a financial drain on the hospital, but they keep him around
1. because he is a genius
2. he has tenure
3. it’s a teaching hospital
4. his reputation brings in VIP patients and raises the prestige of the hospital.

The show is well written and pretty amusing if you’re a medfag, but the showrunners have said a lot of inspiration was taken from Sherlock Holmes (House and Wilson, Holmes and Watson). He’s less of a doctor and more a slightly autistic guy playing detective. Why his character would even become a doctor in the first place instead of a detective or FBI agent is beyond me.

If you’re in the US there are much better ways to make money. The cost of medical education is outrageous, along with getting paid pennies for residency. If you’re half as devoted to a different career, you could easily be in a leadership position in finance or banking by the time a new doctor would be finishing residency and making just as much money with none of the debt, to say nothing of the attrition rates or getting matched into a poorly compensated specialty.

And unless your family is already wealthy (or you somehow secure a full ride scholarship through both undergrad, med school, and a specialization) you will wind up with somewhere between $200 - 500,000 USD in student debt. The interest alone (assuming federal rates) is $14,000 - 35,000 a year without even touching the borrowed amount. 200k a year puts you in the 35% bracket, which drops you to 130k takehome, and that doesnt account for 401k or any employee benefits costs. Add in malpractice insurance, cost of living, a car, a home, and then that several thousand a month to the student loan.

And that assumes a guaranteed 200k salary.

It's not that difficult to become a nurse, it can be a pretty bullshit job in the hospital though. Everybodies problems fall on you, from housekeeping to MD's and you have to solve it. You're always available for family to bitch at and. You have to wipe asses and as a male, lift dead weight until your back breaks. Furthermore, at some point hospital budgets will break and you will be understaffed and overworked for a year straight. If you don't have a passion for people or helping people out it will break you in the hospital.

You are in a unique position to develop relationships with all hospital staff and patients while getting to learn a lot of hands on skills. If you have good interpersonal skills and are quick to think on your feet you'll be fine. It's just about figuring shit out, managing egos, and being vigilant. Just don't step out of your scope of practice and follow your MD's lead.

The jobs available for you are shitty unless you were just an older nurse who got his ADN back when that was the standard and have years of experience behind your resume. No reason to be an LVN, you'll be paid less for a job you do as well as BSN's and have no room for advancement unless you get your BSN.
A BSN means room for advancement, an ADN or LVN just means your stuck at the bedside for life. Also, BSN's are more prepared coming out of college because of more clinical rotations, sims, and capstone rotations.

First of all that's the fucking doctors job to give them that news, that's why they get paid the big bucks and they know prognosis's better than us.

But unfortunately it often falls on us. I had to explain to a pt that what he thought were cysts that we biopsied was actually a tumor that was part of his diffuse metastasized cancer. I calmly explained there was a strong likelihood of him dying.

If your in that situation just tell them calmly and firmly exactly what you know. If they ask you questions you don't have the answer explain to them that you don't want to provide them unreliable information. Use words like death, dying, cancer, etc. etc. don't dance around the subject. It's just like when a pt dies you tell the family "they have died" not "they've moved on".

And just as a reminder, that's the fucking doctors job. I fucking hate MD's that dance around code status's just because they don't want to have a difficult conversation

Maybe this is a place to get some advice.
I have autism and adhd and for the past 5 years or so increasingly bad depression, which has rendered me completely passive at this point, melted my mental capacity and general energy and has got me inching closer to suicide every day (not that I want to die, just to stop feeling so fucked 24/7). I also have chronic gut pain due to constant gas retention which I think is caused by some kind of failure in the coordination between my brain and my gut, which also gives me a headache and makes me feel like I have sort of a permanent hangover. At least I know for certain there's nothing physically wrong with my gut due to a recent colonoscopy.
Today my GP wanted to prescribe me sertraline (also known under the brand name zoloft I think), but after reading about it the risks seem way too fucking high.
My instinct says fuck no to antidepressants, but I'm pretty much at rock bottom right now (although I'm sure I could sink even deeper. It can always get worse as I've found).
Anyone have any experience with antidepressants? Would the fact that I have autism have any influence on the risks I'm taking by using it?
Are the risks worth it?

If you’re as depressed and horrible off as you day then there is literally no risk. Side effects of sertraline are negligible in 99% of cases. You need a psychiatrist and you are in no position to determine the correct prognosis or treatment methodology based on your post.

It's worse at night once I fall in to the irrational twilight sleep, I'll wake up with my heart racing thinking about call alarms and the fact that I forgot to advance my patients morning diet to full for the MD. The problem is I'm in the medical field which makes me feel drug seeking and I've always coped well. From the outside looking in I'm well adjusted with friends, successful in school, and successful in my professional life but I'm tired of dealing with the anxiety day in day out and just want some fucking benzos to get me through the bad days

Went to my pcp and told him straight up the story and he basically said, "well it sounds like you have underlying generalized anxiety disorder but you cope well so here's some trazadone to help you sleep, if it doesn't work call me back and I'll give you some ambien"

bend you over and fuck that tight pussy raw for an hour, feeling your ass sticking and flapping on my thighs on and on... on and on... everyday a new day to try new ways to fuck every orifices your hody provides. Sucking and biting those petite titties and losing conciousness on your lips.

Had a root canal Tuesday, and had severe pain develop Thursday night. Went back to the dentist yesterday morning and got started on penicillin. Now have swelling of the face in the area near where to root canal was done. Needed the root canal due to an infection that had apparently built up over multiple years due to the tooth having a second tooth form inside it originally. Should I consider a trip to ER?

Best: pathology, radiology, most surgery, and neurology.
Worst: anything that involves the need to outwardly communicate empathy to others, I guess. Psych, family med, pediatrics, oncology (except rad onc).

The current consencus is that the notion is bunk for serotonin/depression and accurate for dopamine/psychosis/schizophrenia.
