My girlfriend has irregular(she basicly never hss regular ones) and very painful(she can't live without her ibuprofen) periods although she says she is healthy and fertile as confirmed by a gynecologist. How concerned should I be about this? I really want to have kids later. How likely is she to have fertility problems or trouble getting pregnant? Can her periods be normalized? We're in our early twenties by the way.

Just get her pregnant, it will cure her period pains and give her something real to complain about. The longer you wait to knock her up the higher the chance of getting a retard or a freak.

Is she on a birth control pill? And does she use it to “skip” her periods sometimes?
