There already has been a paper published which suggests we could be looking at the accretion disc from above with the poles (and therefore the astrophysical jets) of the black hole pointed right at us https://arxiv.org/abs/1901.06226
If EHT confirms this then it could have quite large implications. The scariest one is that the black hole acts a a filter for life in this galaxy, explaining the Fermi paradox. If the jets periodically become active then they would be in the galactic plane and potential destroy planets containing life. Also I would imagine there would be a lot of questions raised about how the black hole ended up on its side.

Regardless of all that, the image of the event horizon is going to have huge implications for our current understanding of physics. Depending on whether it conforms to our expected models it could either reaffirm or deny the theory of relativity.

Actually that is mostly solved, since big galaxies like the Milky Way form by merger and the angular momentum adds up during the merger.
A good example of this is the Polar Ring galaxy having two separate parts having different angular momentum axis.
