How can I tell when my son is sick for real or just faking it?

Here are some symptoms I've observed over the past 2 years:

-Cough: There's usually 3-4 different patterns for when he coughs each time. For example: loud-short-short, or short-wheeze-short-short.

-Headache: Usually on the right side of the head, near the temple.

-Sore throat: His voice always sounds like an old man whose nasal passages are clogged. Very raspy.

Whenever he says that his body feels cold but his head feels hot, his face is red. He never claims to have a fever, though.
