Would the 0,2% of said society still have 160 IQ or rather 208 IQ? Would this IQ distribute among various intelligences so a surgeon with very high motor skills could perform a more precise cut than a remote controlled cutting edge extension of arm can do today?

Now a surgeon with a better control of his body is a desirable trait that is probably objectively positive to the society regardless of his "better" opinions. If we are going wild with your hypothesis I just can't empathize with a person who has 208 IQ. That's so far away from even today's genius. My IQ is 152 and all of my close friends are very intelligent and it's not just because average person wouldn't "get me" but because (and this is maybe even more important) the emotion doesn't carry as much weight to them. This would create even deeper valley between the privileged few and the rest because you can still be a factory worker at 130 IQ and in fact you are needed as a factory worker, not everyone can be a creative flower. And so the ultragenious few, detached even more from the general population feeling all alone in the crowd would have to design a system producing IQ 130 unfulfilled factory workers full of sadness.

Now a more interesting question than this IQ thing is, what a modern society with mandatory single oral dose of 20g of psilocybin mushrooms or vaporized 80mg of N,N-DMT as a rite of passage into adulthood would look like? What if cannabis instead of alcohol was legal?
