Does the body have a set internal clock? I've heard about the Circadian Rhythms and the research by Nathaniel Kleitman, but I've also seen Siffre's work on how without time cues, the human body goes into a 48 hour cycle rather than the 24-25 one advocated by Kleitman. Greatly appreciated if someone can shed some light

Diagnostic tests for ADD include standard psychological testing (IQ, abstract and concrete problem solving, behavioral observation, etc) and a sleep-state Electroencephalogram (EGG). These are complimentary methods: the EGG will confirm the psychology.

I'm sure Positron Emission Tomography (PET) profiles have also been built, but PET scans are very expensive and mainly confined to research. They're not necessary for clinical diagnosis.

A gold-standard diagnosis will use both psych testing and the EEG.
A good diagnosis will include psych testing and an experimental course of medication, followed up by another diagnostic to see if it's working.
A poor diagnosis will be a quick questionnaire from a family doctor who then just writes you a script and says "come back for more if that works out for you."
