Theoretically, the atom, as said by the above poster. Practically, it'd be a miracle to control anything smaller than macromolecules.

If you mean to say "what would the building blocks be made of", that's out of my field. Probably a bunch of raw elemental materials in a non-reactive liquid for the nanomachines to move in..

Yup. You gotta have enough of each element to make what you want to make, and some elements are far too volatile. Can't use gases either.

Sure. Take some biochemistry courses and you'll see that the human body (hell, any lving body) is the most advanced nano-robotic device in existence.
