Why? If you go to all the trouble to escape Earth's gravity well, why descend into another one? O'Neill cylinders are planets as humans would design them, with gravity where we want it, but not where we don't. You don't need huge rockets to leave an O'Neill cylinder. You just take an elevator up to the axis, then a train along it to either end cap where the docking hubs are. Presto, you're in space.
