Research and good LoR. A focused SoP that demonstrates a good fit with the faculty you're interested in will also get you noticed as well. You'll get interviews like that because they'll pass your application to the POI if they're hiring that season.

>Is it possible from an okay state school for undergrad?
YES!! I go to a shitty state uni and got interviews with 3 POI's at different top 10 unis for my field. Got into all of them with offers of guaranteed full funding + GRA. How? I had undergrad research experience that was directly in line with what they do and my 3 LoR were fucking god-tiered (my research profs actually let me read their letters to make sure that they were perfect for the programs I applied to). Immaculate letters will get you into places you never dreamed of being able to go to because profs in their respective fields all know each other.

Don't worry about having publications either (I had none), nobody really expects that at all. GRE's are dogshit too. I think I scored like a 158Q when I took it, was too busy doing research to study lol. Despite what the autistic nobodies on 4chan will tell you, standardized tests and numbers say nothing about your research ability. Your GPA and the GRE are a screening factor; clear the minimum threshold to get your app looked at, and then nobody will ever look at them again [source: I worked for profs in adcom].

Let your LoR and research do the talking nigga, not the name of your school or your numbers on a piece of paper. Get to work.
