Maybe when he went to the hospital just recently they amputated the toes since they were the actively gangrenous parts of his leg that needed to go. However when he went there they found that a lot of his foot/lower leg was dead and was in the process of becoming gangrenous, which would need to be amputated (cannot revive dead tissue) but has not yet become gangrenous.
