Proto-humans had minds that operated via a dichotomous structure: a speaking part and an obeying part. These "humans" lacked consciousness and existed before the ancient greeks. Then, almost suddenly, consciousness explodes and people like Socrates, Diogenes, Aristotle, Plato, et al. exist. This rapid onset of introspective, conscious people is due to an evolutionary breakdown of the bicameral mind --- an ouroboros of the speaking and listening minds co-existing.

Schizophrenics are people for whom the division between the two parts of the bicameral mind are still in tact. They hear voices and are incapable of recognizing the voice as their own. Though these people without consciousness who lack schizophrenia also exist today, and are the NPC of today's world, who don't introspect at all.

In short, only some people are conscious.
