Misplacing things isn't destroying them. Having no means to improve it's archivql or storage isn't desteoying. The Viking Mars mission data magnetic tapes have been slowly degrading and that data has been lost over the decades as well because there was no funding for a project to convert that data onto new media forms. Just an example.
Doing things right costs money, money that sometimes just isn't allocated towards NASA in sufficient amounts to properly store and maintain organized archives for the future generations.

This a very good question which I have also wondered since I was a little boy. It really is strange how that design would not have been eliminated through UTI infections and subsequent death. So strange.
However, the design isn't unique to humans, all other apes have such a layout, and pretty much all land mammals have their anuses extremely close to their vulvas. Quadrupeds even have their anus directly above the vagina, so as they excrete fecal matter, it directly ejects said fecal matter just above the vaginal opening.

If all medication has failed, and the person is still a danger to himself and/or others, isolation is needed. In some cases, prison is needed. Prisons are full of murderous psycopaths, after all.
