What would happen to someone stuck in a prison cell together with a highly radioactive source?

1) Yeah, duh, they die. But how fast?
Even the worst cases of death by radiation I've heard about have the individuals removed from the proximity of the source

How likely is that going to happen when a meltdown occurs? Whats the most dangerous thing about the meltdown, when the fuel doesn't enter the earth and gets stopped at the concrete? Is the steam dangerous, because it can lead to an explosion?
