There are a lot of good tutorials, mostly on Deviantart. But it is mostly just choosing a or a few formulas and tweaking the values on them until you get something you think looks decent.

Depends on the specific image. To a fairly far extent, it is human artwork. The computing creates a fractal universe which can be changed a great deal to the creators liking. All color, camera placement, and other effects are entirely non computed.
