It is to do with market structure.

Let's take a standard market.

5% of people want to eat poo
10% of people want to piss on people
15% of people want to be slapped hard in the face with their partner cums.

None of these want what any of the others want.

So if you're writing a pirate adventure novel you make the sex "off screen" so people can insert the poo eating urinal brutality, or the soft hearted love scene.

If you're writing piss porn you've accepted that you've limited your market and you focus on writing about forcing OP to lay down in the public house toilet by kicking his ankles, and then urinating on his face.
