Can you count? Count. If your mind wanders off, don't panic. This is natural. Everyone has a wandering mind. External guidance helps you refocus. Aka YouTube guide, podcast, etc. Whatever you can use to get the mind back into counting.

That's true for almost everyone. The thoughts come and you let them pass, don't identify with them, don't let them take you on a trip. Just be a cold motherfucker staring from within. Even experienced meditators or "enlightened" people aren't empty of spontaneous thoughts, they just don't get dragged around by them.

Yes meditation is mentally painful for me to. Everytime you start thinking be come aware of the thinking and label it, say in your head "thinking thinking" then return to watching your breathes or counting. When it wanders again do the same thing. When you hear a sound say "hearing hearing" and bring it back to the breath. When you have an itch say "itching itching".

Buddhists say if your mind wanders 10 000 times bring it back to focus 10 000 times. You are successfully meditating if your mind wanders and you bring it back, that is like 90% of the practice.

meditation isn't on itself an excercise about focusing, focus meditation is called shamata, but you also have vipasana meidtation in which you want to cultivate awarness, so you don't try to fight your thoughts but be aware of them, of the way the move and manifest, a lot of more advances techniques mix both methods
if you get fixed on the focus part you'll just accumulate stress
