Who wrote the book lit? Zampano, Johnny, or Pelafina?

Who's the true author and who are just characters?

Depends which "level" you mean.

The introduction and the sidenotes are obviously written by Johnny.

I'm not so sure about the main story (the scraps he found). A lot of hints can be found that indicate that Pelafina wrote at least parts of it.
Check the letters at the end. Also you can find a tick at the bottom right of one page. The tick was a secret code between Johnny and Pelafina if you remember.

Let me explain; I think Johnny Truant made the entire thing up, except for his bits. Zampano, The Navidson Record, even Pelafina's letters, are all written by Johnny as he loses his sanity.

Look at how Johnny's described as always telling stories; Lude constantly puts him on the spot, his mother comments on it in the Whalestone letters. The letters also have a message about Zampano, who his mother could never have known. So I think the entire book is Johnny writing through a psychotic episode.

I don't know if Johnny as a young child would have been able to write about the house like that. Could be possible that Pelafina created Zampano as an extension of her guilt, Zampano's hinted at losing a child and being a generally depressed sort of guy. And JT just adds those sidenotes and his sections.

Johnny's used real people in made up stories though, I think. Lude can still exist and he can just be the excuse Johnny has for "finding" Zampano.

It fits Lude's personalty of dragging Johnny into things.
