How do some writers write so well from the mindset of people very different to them? (Rapists, pedophiles, mentally ill etc)?

Usually people say you write what you know, but certain writers manage to make vivid internal narrations from people like the above that make them sound believable and normal.

Is this a natural empathy or a learned skill, do you think?
