you will naturally remember the relevant ones and the ones you are interested in as well as be reminded of all of the words you forgot. its old fashioned advice, but it does work

there are two basics you need to practice: articulation and pronunciation
articulation is how complete you form sounds. Good articulation is necessary regardless of dialect or accent
pronunciation is how it sounds. strive for a general atlantic accent in english

then you just need to train. Read texts out loud (you'll be surprised how tiring it is) to get a feel for monologuing
then you need to exercise speaking on the fly. Best thing is to do a hobby and explain what you are doing
dont worry about vocabulary. its better to be able to speak in a simple and concise manner than to rape a thesaurus

It causes me to believe you may be in need of some heavy-duty therapy such as direct intervention by a benevolent agency that is more interested in your internal affairs than I could ever be. When it comes to writing, I am actually a fucking genius and can articulate anything and everything in an eloquent and impressive way. When it comes to speaking though I just run out of steam lol. Without the time to slowly ponder and formulate my arguments, I just end up stuttering and "ummm"-ing my way into a half-convincing explanation and that's that.
