You have tiny hear cells in your cochleae (the snail shell-looking things in your inner ear) that bend in response to vibration (i.e. sound) and produce electric signals your brain interprets as sound (the frequency depends on where in the cochleae the hair cells are located). Loud sounds can permanently bend the hair cells, so they will either not respond to vibration (hearing loss) or they will produce a constant signal (tinnitus).
Note that these cells do not regenerate, so any hearing damage is permanent, so even minor damages accumulate over your lifetime until it's enough for you to notice it.

I have a very slight tinnitus because of my jaw. Sometimes it gets worse, but I have been able to get used to it with time. Most of the time I don't even notice it; unless something like this thread reminds me.

If it's not from loud noise, then it's probably curable. Your brain can believe it's picking up signals, in the same way some people feel itching where an amputated limb should be. There are gadgets to retrain your brain to stop hearing it, the same way there are gadgets to retrain your brain that your arm isn't there. Basically you have a noise box that you tune to sound like the noise signal your brain is getting, and then turn down the noise on the machine. Eventually your brain starts connecting turning down the box to the signal lessening, and then you can just turn it off and not turn the machine back on and hey you're cured.
On the other hand it can be a couple other things (blood pressure problems being picked up by your ears, jaw problems from wisdom teeth or poor alignment, terminal brain cancer) but those are pretty rare compared to your brain misinterpreting signals.
