I said "basically only ever," the story begins with the two brothers, then they get separated. Yet between two characters you have trouble differentiating who is meant by the word "he" when the other character is named?

No, you're not that dumb, you're just stubborn, perhaps that it isn't fitting itself absolutely to your syntactical wet-dream. At the very least you ought to read to the end of the first story arc in which Billy goes to Mexico. He's entirely by himself, not counting his "pet", and that segment will give you a window into McCarthy's style that can not be confused among an array of personages in any way.
