It depends a lot on the format I'm reading.
If I'm reading a physical book I rarely write or highlight anything on them (except for textbooks), so everytime I want to take notes I write them on a notebook.
If I'm reading on my ereader, I just highlight the text and make an annotation, same for PDFs.
I also keep a TiddlyWiki where I upload my notes and make connections between them.

My bookmark is an index card, and whenever I come across a sentence that gives me some insight or summarizes the author’s position I underline it and write the page number on my index card. Then later that day or the next day I check the numbers on the card and prune the underlininings, and then write the ones I want to keep in a notebook. I like this because it doesn’t interrupt the flow of the reading experience, and it forces me to read the most important ideas several times, so I have the opportunity to internalize them.
