The body without organs is how you look at a body and rather than seeing it as containing multiple organ systems, to you as the person with the body, it is a single object. Rather than having a respiratory system, which deleuze calls a breathing machine, you can see the lungs as part of what's in your body, the BwO is when you deterritorialise or oedipalise- meaning that you see things as a rhizome, something with no beginning or end.
So consider whether you have a penis or a neovagina, the content doesn't matter as it has a purpose and is part of your body. I know it's quite hard but it all fots into place when you see the rhizome.
