Pretty much every book with a live action adaptation (I Am Legend, The Lord of the Rings, The Terror, etc).
I think pic related is among the most bizarre covers I've seen.

Has anyone ever seen any exception to this rule?

Also, that stupid Moby Dick cover with the picture of the boat on the surface and the big whale nudging it and you can see under the water is one of the worst covers ever and it's a well-known edition.
