How to I stop being a /thinker/ and become a /writer/? I'm tired of just imagining my world and stories but I'm terrible at bringing them to life

Start writing; everyone needs a cornerstone on which to build. Get some books about writing and start from there. The most important thing is that you write, always, no matter if what you produce is bad or not, JUST WRITE. The rest will come by itself flowing gently down the stream of practice

If you're a thinker, you surely make up loose phrases and aphorisms. Write them down on a notebook or on your phone, and, once inspiration hits, you can build up on them or combine them or whatever you feel like doing with them. But make sure to take note

You're not going to like this answer, but it needs to be said. Pursue a creative writing bachelor's degree at a 4-year university and then use what you learn to get yourself into an MFA program. Not every writer needs the the help of an MFA program or even of a university at all, but clearly you do and there is no shame in that.

BUT of course, you are mentally going to make excuses for why university programs are bullshit as well as refuse to do anything productive that would actually improve your writing.

This, but I don't know if I would go as far as doing a whole degree. I used to be the same as OP, but just taking a few classes got me in the habit of writing. I would try that and if you're into it then go for the degree, but if you're in the US look for those fully funded MFA programs.
You need deadlines and the fear of being embarrassed and classes are great for that. I don't think I learned that much from my professors but I learned a lot from panicking before workshop days.
Also, it's been said a million times but it's true: you need to read widely and read every day.
