I think it has some partial truth, generally speaking but it over exaggerated.

I think you have to be much lower to not understand someone talking at their level (eg a convo between 140-140).

But a 140 should be able to simplify their ideas enough for lower IQ people to understand. I think this meme is partly spread by high IQs or pseud IQs because they are so caught up in the details and vocabulary, that they themselves don’t actually understand the ideas at their most fundamental/base levels.

IQ is statistically valid, but in terms of it's predictor of success or it's validity and reliability for neurodivergent people, it falls short. Firstly, those in the upper range of intelligence experience a communication barrier with regular folk. This in effect contributes to social isolation and ostracization, which in most cases prevents highly intelligent people from reaching leadership positions (as communication is key; those in the 120's are over-represented in leadership positions as they are still capable of communication while being smart enough to fill in for the roles of teaching and governing). To put it simply, IQ isn't a great predictor of success because things don't work meritocratically with regards to intelligence. IQ doesn't also work well for those in the upper-range because, as your intelligence increases, so does the likelihood for deficiencies (ie ADD/ADHD, depression, autism, aspergers (if that's still a thing), depression, anxiety, twice exceptional people, etc). For neurodivergent's, the normative unitary structure of the brain turns against them. For twice exceptional people, as an example, a certain aspect of intelligence may be specialized while de-emphasizing the other parts. In my case (which doesn't happen as often), my verbal intelligence is miles ahead of my non verbal. This discrepency doesn't translate well for traditional notions of intelligence.
