Way is singular and the subject of to make.
Should read:

The way he structures his sentences makeS him sound like a 5 year old.

or

The wayS he structures his sentences make him sound like a 5 year old.

Since way is singular, why do I need an S at the end of make?

Same reason one says "he makes" and "they make".
It's just the way English works.
