How do I improve my English?

Im not a native speaker, but i'd say that I have a decent grasp on the language. I pretty much have no issue reading or listening and comprehending everything. I can write decently too.
Speaking it is a different situation... I often freeze mid-sentence, forget what word to use and I have terrible accent. I understand that the best way to master it is to use in on a daily basis in everyday situations, but im not from an English speaking country so thats not an option...
I guess this is more of a question towards the non-american part of the board, how did you guys perfect your vocabulary and accent?
