Under the Volcano is perfect. The guy is steadily killing himself with alcohol, and he knows it, and everyone around him knows it, but he never stops. One of the most tragically beautiful books I've ever read.
I don't think anything could fit this better than when he breaks down in front of a Mary shrine and just begs for help. "Deliver me from this dreadful tyranny of self."
