The first step isn't a huge problem, it's the trick of saying, "next, same scenario, except it's a fat boy and you can push him to stop the trolley", to which a lot more people will switch to saying no, at which point the teacher will point out the supposed hypocrisy of being fine with pusing a lever but not being fine with pusing someone directly.

I don't think the Nazi ended up using the chambers because it was too hard to get people to shoot Jews, it really isn't, I think it had more to do with doing it quickly.
The next step is usually the surgeon dilemma, you have a healthy person and 5 dying dudes, you can save them all by harvesting the organs of the healthy dude, killing him. Even fewer people say yes, obviously.

You are killing someone just to save a numerically bigger number of people.
Would you be able to push someone in front of the train? if anything not moving in real life situation or prioritizing those who are emotionally close to you regardless of "cost" is what happens.
