Once upon a time, survival on a societal level was somewhat challenging and it was important that everyone pulled their weight. This means that every capable person had to do work that contributed meaningfully to survival, had to reproduce, had to take care of themselves, etc. Religions (at least the successful ones), as the primitive rulebooks of society, forbade people to act contrary to this. They also encouraged clannishness as a tool of societal cohesion and competetiveness.

Nowadays, at our current state of development, clannishness isn't really beneficial, and society can support a lot of people who aren't pursuing an optimal survival strategy. So people have the chance to pursue careers like literature or astrophysics, which have no tangible and immediate benefits in terms of survival. People are also free to be gay, or fat, or trans, without being insupportable burdens on society. That's why we don't bother to execute them in the name of Allah, or Jesus, or whoever the fuck, any more; because that rulebook is out of date and inapplicable to our modern society.

You can still hate on trans people, or gays, or fatties (fuck knows I'm repulsed by them myself), if you want. But you're fighting the tide. We have no compelling reason to limit those kind of personal choices any more, so identity politics will carry the day. 'Mentally ill' in this case is just another way of saying 'out of tune with society'. In a world of atheists, a Christian would be mentally ill. Once upon a time, yeah, trans people were mentally ill, because their reality was out of whack with society's. But now society is catching up. You can still view them as mentally ill if you like, but increasingly that's going to make you the crazy one.
