I start with either the corner hug or the fulcrum while lying in bed, my back supported by my cover in an angle < 45°, my head supported by my cushion lying on top of the cover. Then after a while, that tires me and I lay down completely, rotating so my body and my view are mostly parallel to the mattress, shifting to either a left- or right-handed version of the clotheshanger with a 90° angle between the open pages, half of the book comfortably supported by the mattress, which half being determined by whether the page I was reading before altering my posture is found on the left or the right side of the book. Then, when reaching the next page I either rotate my body to the other side or I stuff the cushion under my head, elevating it in a slightly diagonal way, enough for me being able to read the horizontal page.

Doorstop, but I don't use my thumb to hold the middle inside of the book. I use my thumb to hold the left side, and I use the ring finger and pinky to hold the right, all on my left hand. The index and middle finger stay outside on the cover. It's kinda awesome and doesn't stress my hand out that much.

I use Corner Hug and Doorstop for mass market paperbacks and normal-sized paperbacks that aren't thick as fuck, and Fulcrum for IJ-tier thick books (I often stretch my right hand to wrap fingertips around the spine, instead of how it's shown in the pic). I only use Clotheshanger if I'm trying to grab something (drink, pencil, another cigarette)

On a bench, bus, or anything without a surface: clotheshanger, maybe holding the pages apart with my left hand depending on the thickness of book/how far into it I am

Laying down: start with fulcrum with my head/shoulders propped on pillows, move pillows down and lay like facing my walls, switch to corner hugs with one side of the book resting on the bed depending on which page I'm on, I roll over every page to keep from getti g too tired and falling asleep

Depending on how thick the book is, or how tight the binding is, I prefer the door step as I don't like to get fingerprints all over my pages. But if that makes it difficult to keep the book open to my current page, then O opt for corner hugs.
