Easy:

Pulling the lever would be explicit murder, so you have to let the train press on toward the five; however, you can have fun with it and make it a better experience for four of the people. What you do is you run up to them, choose the most attractive one, and just start fucking his/her throat like wild. Tell the rest, "We may die, but at least you won't die while being orally raped." As you hear the sound of the train crushing its first victim and feel yourself jizzing deep into your victim's throat, it will be the most intense experience of your life. when the variable action is literally as simple as pulling a lever and you are the only one around to do so, the burden falls upon you.

the situation does not change in any functional way when you reverse the positions of the 1 person and the 5 people other than that the position of the lever has the opposite effect

here's some food for thought:
that train is on a schedule and other trains are as well, presumably made to accommodate the schedules of each other.
that being said, changing the direction of that train would mean it is going somewhere the rail company is not expecting it to go.

best case scenario from pulling the lever: one person dies and the train is stopped at the wrong station, causing confusion

worst case scenario from pulling that lever: one person dies initially, then a collision occurs. maybe with a passenger train or one carrying toxic cargo. i cant say or certain how many deaths that would cause, but it would sure as hell be more than 5
